【摘要】：背景:膝關(guān)節(jié)單髁置換術(shù)(unicompartmental knee arthroplasty,UKA)是治療膝關(guān)節(jié)內(nèi)側(cè)間室骨關(guān)節(jié)炎的有效方法,但對于其初期療效尚存在很大爭議。目的目的:探討UKA和全膝關(guān)節(jié)置換術(shù)(total knee arthroplasty,TKA)在治療膝關(guān)節(jié)內(nèi)側(cè)間室骨關(guān)節(jié)炎的初期臨床療效,為臨床手術(shù)方案選擇提供參考依據(jù)。方法方法:回顧性分析2011年1月至2016年1月采用OxfordⅢ單髁系統(tǒng)治療膝關(guān)節(jié)內(nèi)側(cè)間室骨關(guān)節(jié)炎患者45例,男21例,女24例,年齡52~78歲,平均(64.5±7.5)歲。并隨機(jī)選取同期行TKA并得到隨訪的膝關(guān)節(jié)內(nèi)側(cè)間室骨關(guān)節(jié)炎患者45例,男18例,女27例;年齡52~75歲,平均(63.2±10.0)歲。記錄兩組患者的手術(shù)時間、術(shù)中出血量、輸血率、術(shù)后引流量、住院時間,以及術(shù)前、術(shù)后7 d、術(shù)后6個月、末次隨訪時的膝關(guān)節(jié)美國特種外科醫(yī)院(HSS)評分、疼痛視覺模擬(VAS)評分、膝關(guān)節(jié)活動度(ROM)。結(jié)果結(jié)果:全部獲得隨訪,隨訪時間為6~50個月,平均(25.5±10.5)個月。均無假體松動下沉、脫位及對側(cè)間室病變。兩組患者手術(shù)情況及術(shù)后恢復(fù)功能比較,UKA組在手術(shù)時間、術(shù)中出血量、輸血率、術(shù)后引流量、住院時間、術(shù)后7 d及6個月隨訪時HSS評分、VAS評分、ROM明顯優(yōu)于TKA組(P0.05);末次隨訪時,兩組患者的HSS評分、VAS評分及ROM比較無統(tǒng)計學(xué)差異(P0.05)。結(jié)論結(jié)論:UKA治療膝關(guān)節(jié)內(nèi)側(cè)間室骨關(guān)節(jié)炎的初期療效優(yōu)于TKA,且具有手術(shù)時間短、術(shù)中出血量少、術(shù)后恢復(fù)快等優(yōu)點。
[Abstract]:Background: single condyle replacement of knee joint (unicompartmental knee arthroplasty,UKA) is an effective method for the treatment of medial interventricular osteoarthritis of knee joint, but the initial curative effect is still controversial. Objective: to investigate the initial clinical effect of UKA and total knee arthroplasty (total knee arthroplasty,TKA) in the treatment of medial interventricular osteoarthritis of knee joint, and to provide reference for the selection of clinical surgical scheme. Methods: from January 2011 to January 2016, 45 patients (21 males and 24 females, with an average age of (64.5 鹵7.5) years) with medial interventricular osteoarthritis of knee joint were treated with Oxford III unicondylar system. 45 patients with medial interventricular osteoarthritis of knee joint, 18 males and 27 females, with an average age of (63.2 鹵10.0) years, were randomly selected and followed up with TKA at the same time. The operation time, intraoperative blood loss, transfusion rate, postoperative drainage, hospitalization time, and (HSS) score of American special surgical hospital of knee joint before operation, 7 days after operation, 6 months after operation and at the last follow-up were recorded. Pain visual simulation (VAS) score, knee motion (ROM). Results: all patients were followed up for 6 脳 50 months, with an average of (25.5 鹵10.5) months. There were no loosening and subsidence, dislocation and contralateral interchamber lesions. The operation time, intraoperative bleeding volume, blood transfusion rate, postoperative drainage rate, hospitalization time, HSS score and VAS score were compared between the two groups in operation time, intraoperative bleeding volume, blood transfusion rate, postoperative drainage volume, hospitalization time, HSS score and VAS score at 7 days and 6 months after operation. ROM was significantly better than TKA group (P 0.05). At the last follow-up, there was no significant difference in HSS score, VAS score and ROM between the two groups (P 0.05). Conclusion: the initial curative effect of UKA in the treatment of medial interventricular osteoarthritis of knee joint is better than that of TKA, and has the advantages of short operation time, less intraoperative bleeding and faster postoperative recovery.
