【摘要】：目的探討不同麻醉方式對(duì)老年手術(shù)患者術(shù)后感染發(fā)生及疼痛狀態(tài)的影響,以尋找更合適老年手術(shù)患者的麻醉方式。方法收集醫(yī)院2013年1月-2015年1月骨科及腫瘤科收治300例行手術(shù)治療患者,分為3組各100例,A組給予吸入強(qiáng)化全麻,B組給予靜脈強(qiáng)化全麻,C組給予硬膜外麻醉;觀察三組患者手術(shù)后至拆線前感染情況,包括感染部位及感染例數(shù);術(shù)后0、6、12、24、48h視覺模擬疼痛(VAS)評(píng)分,以了解術(shù)后不同時(shí)間患者疼痛情況;同時(shí)觀察三組患者麻醉相關(guān)并發(fā)癥發(fā)生情況。結(jié)果 A組術(shù)后15例發(fā)生感染,B組術(shù)后10例發(fā)生感染,C組術(shù)后3例發(fā)生感染,C組術(shù)后感染率低于其他兩組(P0.05);感染部位發(fā)生率對(duì)比,差異無統(tǒng)計(jì)學(xué)意義;三組術(shù)后0hVAS評(píng)分對(duì)比,差異無統(tǒng)計(jì)學(xué)意義,術(shù)后6、12、24hVAS評(píng)分均高于同組術(shù)后0h(P0.05),三組術(shù)后同期組間兩兩對(duì)比,差異均無統(tǒng)計(jì)學(xué)意義;三組術(shù)后均未見嚴(yán)重麻醉相關(guān)并發(fā)癥,C組麻醉相關(guān)并發(fā)癥發(fā)生率低于A、B組(P0.05)。結(jié)論老年手術(shù)患者選擇硬膜外麻醉,可降低術(shù)后感染及疼痛狀態(tài),是老年患者理想選擇。
[Abstract]:Objective to investigate the effect of different anaesthesia methods on postoperative infection and pain in elderly patients, so as to find a more suitable anesthetic method for elderly patients. Methods from January 2013 to January 2015, 300 patients in orthopedic department and oncology department were divided into three groups: group A (n = 100), group B (n = 100) and group C (epidural anesthesia). The (VAS) scores of visual simulated pain at 0, 6, 12, 24 and 48 hours after operation were observed, including the location of infection and the number of infection cases, in order to understand the pain status of the patients at different time after operation. At the same time, the incidence of anesthesia-related complications in the three groups was observed. Results infection occurred in 15 cases in group A, 10 cases in group B and 3 cases in group C, and the postoperative infection rate in group C was lower than that in the other two groups (P 0.05). There was no significant difference in the incidence of infection sites between the two groups. There was no significant difference in 0hVAS score among the three groups after operation. At 6, 12 and 24 hours after operation, the VAS score of the three groups was higher than that of the same group at 0 hours after operation (P 0.05). There was no significant difference between the three groups at the same time after operation. No severe anesthesia-related complications were found in all three groups. The incidence of anesthesia-related complications in group C was lower than that in group A. the incidence of anesthesia-related complications in group C was lower than that in group A (P 0.05). Conclusion epidural anesthesia can reduce postoperative infection and pain in elderly patients, and it is an ideal choice for elderly patients.
