【摘要】：目前膽囊切除術(shù)是治療膽道結(jié)石病等膽道系統(tǒng)疾病的常規(guī)手術(shù)流程。但是手術(shù)過程中,當(dāng)醫(yī)生遇到膽囊管過短,或者膽道粘連嚴(yán)重,膽道系統(tǒng)的解剖異常等情況時,容易對膽道結(jié)構(gòu)產(chǎn)生誤識別,從而造成膽道系統(tǒng)的損傷。在本文中,針對上述問題我們提出了一種便攜式熒光顯微成像系統(tǒng),可以實(shí)時清晰地顯示膽囊三角結(jié)構(gòu)的細(xì)節(jié),從而防止膽囊切除手術(shù)過程中對膽道結(jié)構(gòu)誤認(rèn)而造成的醫(yī)源性膽道損傷的發(fā)生。該系統(tǒng)由光源模塊,CMOS攝像頭,樹莓派卡片電腦和5英寸的LCD屏組成。具體地,光源模塊是由690納米和850納米的LED陣列組成,CMOS攝像頭按照時序依次獲取熒光圖像和背景圖像。該便攜熒光顯微影像系統(tǒng)的軟件構(gòu)建是基于樹莓派平臺的Linux環(huán)境使用Python編程,調(diào)用OpenCV庫進(jìn)行圖像處理與顯示。實(shí)驗(yàn)測試中,選擇吲哚菁綠(ICG)作為熒光造影劑,并與商業(yè)Xenogen IVIS系統(tǒng)對比在不同濃度下的ICG水溶液的熒光強(qiáng)度的探測能力。系統(tǒng)的空間分辨率用1951 USAF分辨率標(biāo)靶進(jìn)行測量,系統(tǒng)的動態(tài)響應(yīng)用自動位移平臺來定量評價。最后,我們在小鼠的膽管模型上驗(yàn)證了系統(tǒng)的技術(shù)可行性,模擬進(jìn)行正確和不正確的膽囊切除術(shù)。實(shí)驗(yàn)結(jié)果表明,該系統(tǒng)可以觀察到膽囊管,膽總管及肝膽管之間的匯合處的清晰結(jié)構(gòu),這表明該系統(tǒng)是指導(dǎo)膽囊切除術(shù)的潛在方法。該便攜式熒光顯微影像系統(tǒng)的成本在2000元左右,在資源有限的地區(qū)有很大的推廣潛力和價值。
[Abstract]:Cholecystectomy is a routine procedure for the treatment of biliary tract diseases such as cholelithiasis. However, in the course of operation, it is easy for doctors to mistakenly identify the structure of the bile duct and cause the injury of the biliary system when the gallbladder duct is too short, or the adhesion of the bile duct is serious, and the anatomy of the biliary system is abnormal and so on. In this paper, we propose a portable fluorescence microscopic imaging system to solve the above problems, which can display the details of the triangle structure of gallbladder in real time and clearly. In order to prevent the occurrence of iatrogenic biliary tract injury caused by misrecognition of biliary tract structure during cholecystectomy. The system consists of a light source module, a CMOS camera, a raspberry pie card computer and a 5-inch LCD screen. Specifically, the light source module is composed of 690nm and 850nm LED arrays. The CMOS camera acquires the fluorescence image and the background image according to the time sequence. The software construction of the portable fluorescence microscopic image system is based on the Linux environment of the raspberry pie platform, which uses Python programming and calls the OpenCV library for image processing and display. In the experiment, indocyanine green (ICG) was selected as fluorescence contrast agent, and compared with commercial Xenogen IVIS system, the detection ability of fluorescence intensity of ICG aqueous solution at different concentrations was compared. The spatial resolution of the system is measured with a resolution target of 1951 USAF, and the dynamic response of the system is quantitatively evaluated by an automatic displacement platform. Finally, we verified the technical feasibility of the system on the bile duct model of mice, and simulated correct and incorrect cholecystectomy. The experimental results show that the system can observe the clear structure of the junction between the gallbladder duct, the common bile duct and the hepatobiliary duct, which indicates that the system is a potential method for guiding cholecystectomy. The cost of the portable fluorescence microscopic imaging system is about 2000 yuan, which has great potential and value in limited resources areas.
