漢族與維吾爾族男性胸腰段椎弓根應(yīng)用解剖的CT研究及其臨床意義

【摘要】:目的:CT掃描并測量漢族與維吾爾族男性胸腰段椎弓根應(yīng)用解剖的相關(guān)參數(shù),為臨床應(yīng)用椎弓根螺釘內(nèi)固定提供應(yīng)用解剖學(xué)資料。方法:用CT對120名成年男性受檢者(漢族60例,維吾爾族60例)的胸腰段(T10-L2)椎體進(jìn)行掃描,掃描后的圖像行三維重建,在骨窗窗位下使用瀏覽器中角度及長度測量工具測量并記錄橫斷面椎弓根寬度(TPW),椎弓根軸線長度(PAL),椎弓根橫斷面角(TPA)和椎弓根矢狀面角(SPA)。同時記錄受檢者的年齡、身高。以上數(shù)據(jù)運(yùn)用SPSS13.0軟件進(jìn)行統(tǒng)計學(xué)處理。結(jié)果:漢族組在T12和L1節(jié)段的TPW值大于維吾爾族組,漢族組在T12和L2節(jié)段的TPA值大于維吾爾族組,漢族組在T11和L1節(jié)段的PAL和SPA值大于維吾爾族組,差別有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:本實驗所測量漢族與維吾爾族男性胸腰段椎弓根各徑線及角度部分?jǐn)?shù)據(jù)有差異,任何椎弓根形態(tài)測量所得的均數(shù)值只能為安全的置入椎弓根螺釘提供一個參考。在椎弓根螺釘置入術(shù)前行CT測量相關(guān)參數(shù),并制定個體化置釘方案,能有效的降低因誤置而導(dǎo)致的不必要的術(shù)后并發(fā)癥發(fā)生率。
[Abstract]:Aim: to measure the parameters of applied anatomy of thoracolumbar pedicle of Han and Uygur men by CT, and to provide applied anatomical data for clinical application of pedicle screw fixation. Methods: the thoracolumbar (T10-L2) vertebrae of 120 adult male subjects (60 Han and 60 Uyghur) were scanned by CT. Measurement and recording of transverse pedicle width (TPW), pedicle axis length (PAL), pedicle cross sectional angle (TPA) and pedicle sagittal angle (SPA). By using the middle angle and length measurement tool in the bone window and window position of the bone window and using the middle angle and length measurement tool in the browser to measure and record the transverse pedicle width (TPA) and the sagittal angle of the pedicle (SPA). The age and height of the subjects were also recorded. The above data were statistically processed by SPSS13.0 software. Results: the TPW values in the T12 and L1 segments of the Han group were higher than those in the Uighur group, the TPA values in the T12 and L2 segments in the Han group were higher than those in the Uygur group, and the PAL and spa values in the T11 and L1 segments in the Han group were higher than those in the Uygur group. The difference was statistically significant (P0.05). Conclusion: there are some differences in the diameter and angle of thoracolumbar pedicle between Han and Uighur males. The mean value of any pedicle morphometry can only provide a reference for safe placement of pedicle screw. CT was used to measure the related parameters before pedicle screw insertion, and a individualized screw placement scheme was made, which could effectively reduce the incidence of unnecessary postoperative complications caused by misplacement of pedicle screws.
