【摘要】：目的:研究氨甲環(huán)酸(tranexmic acid,TXA)不同給藥方式對全膝關(guān)節(jié)置換術(shù)(total knee arthroplasty,TKA)圍手術(shù)期總失血量的影響。方法:將70例行單側(cè)初次TKA的患者隨機(jī)分為3組:局部應(yīng)用TXA為A組(n=24);序貫聯(lián)合使用TXA為B組(n=23)。關(guān)節(jié)腔內(nèi)注射等量生理鹽水為C組(n=23)。圍手術(shù)期檢測患者的總失血量、輸血量、輸血率、引流量、下肢周徑、D二聚體、血栓彈力圖。結(jié)果:A、B、C組圍手術(shù)期平均總失血量分別為(541±210)、(456±216)、(731±179)ml;A、B、C組術(shù)后24 h平均引流量分別為(104±44)、(98±48)、(160±60)ml。總失血量及引流量方面C組與A、B組比較有統(tǒng)計(jì)學(xué)差異(P0.05),A、B組之間沒有統(tǒng)計(jì)學(xué)差異(P0.05)。3組患者在輸血量、輸血率、下肢周徑方面結(jié)果沒有統(tǒng)計(jì)學(xué)差異(P0.05)。A組患者中發(fā)生了1例下肢深靜脈血栓,其余患者均沒有下肢深靜脈血栓(deep vein thrombosis,DVT)的發(fā)生(P0.05)。結(jié)論:TXA序貫聯(lián)合應(yīng)用組的總失血量和引流量較局部應(yīng)用組少,但在統(tǒng)計(jì)學(xué)上沒有差異。兩者在輸血量、輸血率、下肢周徑方面結(jié)果沒有統(tǒng)計(jì)學(xué)差異。2種用藥方式均不會增加下肢深靜脈血栓發(fā)生的風(fēng)險(xiǎn)。
[Abstract]:Aim: to study the effect of different administration methods of tranexmic acid,TXA on the total blood loss during the perioperative period of total knee arthroplasty (total knee arthroplasty,TKA). Methods: 70 patients with unilateral primary TKA were randomly divided into 3 groups: group A (n = 24) received local TXA and group B (n = 23) received sequential combination of TXA (n = 23). The same amount of saline was injected into the articular cavity as group C (n = 23). Total blood loss, blood transfusion rate, drainage flow, peripheral diameter of lower limbs, D dimer and thrombus elastogram were measured during perioperative period. Results: the mean total blood loss in group A, B and C was (541 鹵210), (), 456 鹵216), (731 鹵179) ml. at 24 h after operation in group A, B and C, respectively (104 鹵44), (98 鹵48), (160 鹵60) ml. at 24 h after operation in group A, B and C, respectively. The total blood loss and drainage volume in group C was statistically different from that in group A and B (P0.05), but there was no statistical difference between group A and group B (P0.05). 3 groups of patients in the volume of blood transfusion, blood transfusion rate, there was no significant difference between group A, group B (P0.05). There was no significant difference in the peripheral diameter of lower extremities (P 0.05). There was no significant difference in deep venous thrombosis (deep vein thrombosis,DVT) in the). A group (P0.05). Conclusion: the total blood loss and drainage volume in the sequential combination of TXA group were less than those in the local group, but there was no statistical difference between the two groups. There was no significant difference in blood transfusion volume, blood transfusion rate and lower limb circumference between the two groups. Neither of the two methods increased the risk of deep venous thrombosis of the lower limbs.
