【摘要】：背景:研究顯示,微創(chuàng)內(nèi)固定系統(tǒng)和股骨近端防旋髓內(nèi)釘有著相同的生物力學(xué)強(qiáng)度。目的:觀察老年股骨轉(zhuǎn)子間骨折采用股骨遠(yuǎn)端微創(chuàng)穩(wěn)定系統(tǒng)鋼板倒置手術(shù)治療的安全性和療效。方法:30例老年股骨轉(zhuǎn)子間骨折采用股骨遠(yuǎn)端微創(chuàng)內(nèi)固定系統(tǒng)鋼板倒置手術(shù)治療,對手術(shù)時間,切口長度,總出血量(術(shù)中及術(shù)后1d引流量)進(jìn)行記錄。同時采用POSSUM、P-POSSUM評分系統(tǒng),預(yù)測并發(fā)癥、死亡的情況,評估手術(shù)的風(fēng)險(xiǎn)。對術(shù)后并發(fā)癥、死亡患者進(jìn)行統(tǒng)計(jì)并和預(yù)測的情況對比,觀察微創(chuàng)內(nèi)固定系統(tǒng)鋼板倒置手術(shù)治療的安全性。記錄骨折愈合時間,末次Harris評分,觀察其療效。結(jié)果與結(jié)論:(1)預(yù)測POSSUM并發(fā)癥發(fā)生率46.05%,P-POSSUM死亡率發(fā)生率為3.93%,實(shí)際并發(fā)癥和死亡率的發(fā)生分別為13.3%和0%,并發(fā)癥差異有顯著性意義(P=0.05),死亡率差異無顯著性意義(P=1);(2)骨折愈合時間21-32周,末次Harris評分優(yōu)良率83.3%;(3)結(jié)果說明,老年股骨轉(zhuǎn)子間骨折采用股骨遠(yuǎn)端微創(chuàng)內(nèi)固定系統(tǒng)鋼板倒置手術(shù)治療的手術(shù)操作方便,安全性可靠,療效較好。
[Abstract]:Background: studies have shown that minimally invasive internal fixation systems share the same biomechanical strength as proximal femoral anti-rotating intramedullary nailing. Objective: to observe the safety and efficacy of minimally invasive stable system plate inversion in the treatment of intertrochanteric fractures in the elderly. Methods: thirty elderly patients with intertrochanteric fracture were treated with plate inversion of distal femoral internal fixation system. The operation time, incision length and total bleeding volume (drainage flow during operation and 1 day after operation) were recorded. At the same time, POSSUM,P-POSSUM scoring system was used to predict complications and death, and to assess the risk of surgery. In order to observe the safety of minimally invasive internal fixation system plate inversion operation, the complications and death of patients after operation were statistically compared with those predicted. The time of fracture healing and the final Harris score were recorded and the curative effect was observed. Results & conclusion: (1) the incidence rate of complications in POSSUM was 46.05%, the mortality rate was 3.93%, the actual complications and mortality rates were 13.3% and 0%, respectively. There was significant difference in complications (P < 0.05), but there was no significant difference in mortality (P < 0.05). (2) the fracture healing time was 21-32 weeks, and the excellent and good rate of the last Harris score was 83. 3%; (3) the results showed that the treatment of intertrochanteric fractures in the elderly by plate inversion with distal femoral internal fixation system was convenient, safe and reliable, and the curative effect was good.
