3-D打印個性化外固定器在嚴(yán)重脛腓骨骨折的初步應(yīng)用

【摘要】:目的采用3-D打印技術(shù)制造個性化復(fù)位型外固定器,探討嚴(yán)重脛腓骨粉碎性骨折的治療新方法。方法 2015年4月選擇1例脛腓骨嚴(yán)重粉碎性骨折患者納入研究;颊邽槟行,年齡18歲,身高171 cm,體質(zhì)量67 kg。采用計(jì)算機(jī)輔助技術(shù)與3-D打印技術(shù)相結(jié)合,定制個性化外固定支架對其進(jìn)行治療,術(shù)后觀察臨床療效。結(jié)果支架安裝時間約10 min,術(shù)中無需透視,復(fù)位一次成功。術(shù)后雙下肢恢復(fù)等長;復(fù)查X線片示遠(yuǎn)折端向后成角糾正了37°、外翻成角糾正了4°,脛骨骨折端對位對線良好,下肢力線完全矯正,與軟件模擬復(fù)位后模型一致。術(shù)后未出現(xiàn)新的骨折移位,3.5個月后骨折臨床愈合,8個月后達(dá)堅(jiān)強(qiáng)骨性愈合;膝關(guān)節(jié)活動功能良好。結(jié)論應(yīng)用3-D打印個性化外固定器治療脛腓骨嚴(yán)重粉碎性骨折,具有操作方便、個性化程度高、復(fù)位效果好、固定牢靠、復(fù)位無需X線透視等優(yōu)點(diǎn)。
[Abstract]:Objective to study the treatment of severe comminuted tibia and fibula fracture by using 3-D printing technique to make personalized reduction external fixator. Methods A case of severe comminuted tibia and fibula fracture was selected in April 2015. The patient was male, aged 18 years, and 67 kg. tall with a body weight of 171 cm,. Combined with computer aided technique and 3-D printing technique, individualized external fixator was used to treat it, and the clinical effect was observed after operation. Results the stent installation time was about 10 min, without fluoroscopy, and the reduction was successful. After operation, the recovery of both lower limbs was equal length, the X ray film showed that the distal fold end was corrected by 37 擄, the extraversion angle was corrected by 4 擄, the tibial fracture end was well aligned, and the lower extremity force line was completely corrected, which was consistent with the model after software simulation reduction. There was no new fracture displacement after operation, the fracture healed after 3.5 months, and reached strong bone healing after 8 months, and the knee joint movement function was good. Conclusion the treatment of severe comminuted fracture of tibia and fibula with 3-D printing individualized external fixator has the advantages of convenient operation, high degree of individuation, good effect of reduction, firm fixation, and no need of X-ray fluoroscopy in the reduction of tibia and fibula.
