【摘要】：目的:觀察馬黃酊濕敷聯(lián)合改良Donati-Allgower縫合法治療跟骨骨折ORIF術后預防感染、皮瓣壞死的臨床療效。探討跟骨骨折ORIF術后感染、皮瓣壞死的原因,以期為該并發(fā)癥的治療提供可行的方案。方法:將本課題中的52例患者,隨機平均分配成四個組別:單純縫合組(A1組)、單純縫合聯(lián)合馬黃酊濕敷組(A2組)、改良Donati-Allgower縫合組(B1組)、改良Donati-Allgower縫合聯(lián)合馬黃酊濕敷組(B2組)。觀察患者術后感染率、皮瓣壞死率、軟組織愈合時間。結果:改良Donati-Allgower縫合聯(lián)合馬黃酊濕敷組與其他三組分別比較,其皮瓣壞死率、感染率明顯降低,且P0.05,差異較大,統(tǒng)計學意義明顯。改良Donati-Allgower縫合聯(lián)合馬黃酊濕敷組軟組織愈合時間足數(shù)分別與其他三組比較,P0.05,差異較大,統(tǒng)計學意義明顯;改良Donati-Allgower縫合聯(lián)合馬黃酊濕敷組和改良Donati-Allgower縫合組軟組織愈合時間明顯大于其他兩組;改良Donati-Allgower縫合聯(lián)合馬黃酊濕敷組軟組織平均愈合時間足數(shù)分別與其他三組比較,P0.05,差異較大,統(tǒng)計學意義,明顯;改良Donati-Allgower縫合聯(lián)合馬黃酊濕敷組和改良Donati-Allgower縫合組軟組織平均愈合時間明顯大于其他兩組。結論:馬黃酊濕敷聯(lián)合改良Donati-Allgower縫合法在治療跟骨骨折中,可有效降低感染和皮瓣壞死的發(fā)生率。Donati-Allgower縫合法可延長軟組織愈合時間。馬黃酊外用在預防和治療感染方面具有極好的療效。
[Abstract]:Objective: to observe the clinical effect of Marhuang tincture wet compress combined with modified Donati-Allgower suture for preventing infection and skin flap necrosis after ORIF operation for calcaneal fracture. To investigate the causes of infection and flap necrosis after ORIF operation for calcaneal fracture, and to provide a feasible scheme for the treatment of this complication. Methods: 52 patients in this study were randomly divided into four groups: simple suture group (group A1), simple suture combined with wet-compress group (group A2), modified Donati-Allgower suture group (group B1). Modified Donati-Allgower suture combined with wet compress group (group B2). The postoperative infection rate, flap necrosis rate and soft tissue healing time were observed. Results: compared with the other three groups, the necrosis rate and infection rate of skin flap in modified Donati-Allgower suture combined with wet compress group of Ma Huang tincture were significantly lower than those in the other three groups, and the difference was significant (P 0.05). Compared with the other three groups, the number of soft tissue healing time of modified Donati-Allgower suture combined with wet-compress group of Ma Huang tincture was higher than that of the other three groups (P0.05, P < 0.05). The soft tissue healing time of modified Donati-Allgower suture combined with wet compress group and modified Donati-Allgower suture group was significantly longer than that of the other two groups. Compared with the other three groups, the average healing time of soft tissue in the modified Donati-Allgower suture combined with wet-compress group of Ma Huang tincture was higher than that of the other three groups (P0.05, P < 0.05). The average healing time of soft tissue in modified Donati-Allgower suture combined with wet compress group and modified Donati-Allgower suture group was significantly longer than that in the other two groups. Conclusion: the wet compress of Ma Huang tincture combined with modified Donati-Allgower suture can effectively reduce the incidence of infection and flap necrosis in the treatment of calcaneal fracture. Donati-Allgower suture can prolong the healing time of soft tissue. The external application of Malus tincture has excellent curative effect in preventing and treating infection.
