【摘要】：目的分析嚴(yán)重胸腰椎骨折脫位經(jīng)后路長(zhǎng)節(jié)段椎弓根螺釘內(nèi)固定術(shù)后脊柱冠狀位力線、矢狀位力線恢復(fù)不良的情況及其原因。方法分析33例胸腰椎骨折脫位(AO分型C型)病例,均接受后路長(zhǎng)節(jié)段內(nèi)固定手術(shù)。在X線平片上測(cè)量患者手術(shù)前、后傷椎移位距離、脊柱側(cè)(后)凸Cobb角;根據(jù)術(shù)后力線恢復(fù)情況,將患者分為力線恢復(fù)良好組(A組)和力線恢復(fù)不良組(B組),并分析螺釘數(shù)目、傷椎置釘?shù)葘?duì)術(shù)后脊柱力線恢復(fù)的影響。結(jié)果 33例患者中,力線恢復(fù)良好占60.6%(20例),力線恢復(fù)不良占39.4%(13例)。兩組患者固定節(jié)段和使用椎弓根螺釘總數(shù)方面的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);每個(gè)椎體椎弓根螺釘數(shù)目A組為(1.7±0.3)個(gè),B組為(1.3±0.4)個(gè),其差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論嚴(yán)重胸腰椎骨折后路長(zhǎng)節(jié)段內(nèi)固定術(shù)后力線恢復(fù)不良并非少見,固定節(jié)段、螺釘使用數(shù)目、傷椎置釘?shù)葘?duì)術(shù)后脊柱力線的恢復(fù)有重要影響。
[Abstract]:Objective to analyze the bad recovery of coronal and sagittal force lines of spinal column after posterior long segment pedicle screw fixation for severe thoracolumbar fracture and dislocation. Methods 33 cases of thoracolumbar fracture and dislocation (AO type C) were analyzed. The displacement distance of the injured vertebrae and the Cobb angle of the lateral (posterior) kyphosis were measured on the X-ray plain film before operation. According to the recovery of force line after operation, the patients were divided into two groups: group A (good recovery of force line) and group B (poor recovery of force line). The effect of screw number and screw insertion on the recovery of spinal force line after operation was analyzed. Results among 33 patients, good recovery rate was 60.6% (20 / 33) and poor recovery rate was 39.4% (13 / 33). There was no significant difference in the total number of fixed segments and pedicle screws between the two groups (P0.05). The number of pedicle screws in group A was (1.7 鹵0.3), and that in group B was (1.3 鹵0.4), the difference was statistically significant (P0.05). Conclusion it is not uncommon for severe thoracolumbar fracture to recover force line after long segment internal fixation. Fixation segment, number of screws and injured screw have important influence on the recovery of force line after thoracolumbar fracture.
