【摘要】：目的探討右美托咪定對(duì)甲狀腺手術(shù)患者嗆咳反應(yīng)的影響。方法擇期全麻下甲狀腺腫瘤手術(shù)患者118例,男43例,女75例,年齡18~65歲,BMI 18~30kg/m2,ASAⅠ或Ⅱ級(jí),采用隨機(jī)數(shù)字表法分為兩組,每組59例。D組在手術(shù)開始時(shí)持續(xù)靜脈泵注右美托咪定0.4μg·kg~(-1)·h~(-1)至手術(shù)結(jié)束,C組在手術(shù)開始時(shí)持續(xù)靜脈泵注等容量生理鹽水至手術(shù)結(jié)束。觀察患者全麻恢復(fù)期間嗆咳發(fā)生情況,記錄患者術(shù)后24、48h引流量,以及心動(dòng)過速、低血壓、高血壓、心動(dòng)過緩等不良反應(yīng)的發(fā)生情況。結(jié)果 D組全麻恢復(fù)期間嗆咳發(fā)生率明顯低于C組(10.1%vs 30.5%,P0.05),D組術(shù)后第1個(gè)24h[(62.3±26.8)ml vs(78.6±30.1)ml,P0.05]、第2個(gè)24h[(28.0±12.5)ml vs(37.5±18.6)ml,P0.05]引流量明顯少于C組;D組圍術(shù)期心動(dòng)過速發(fā)生率明顯低于C組(3.4%vs 23.7%,P0.05)。結(jié)論術(shù)中持續(xù)輸注右美托咪定0.4μg·kg-1·h-1能夠有效減輕甲狀腺手術(shù)患者全麻恢復(fù)期間的嗆咳反應(yīng),降低患者術(shù)后出血的風(fēng)險(xiǎn)。
