利尿合劑治療混合痔術后尿潴留的臨床研究

【摘要】:目的:對利尿合劑治療混合痔術后尿潴留的臨床治療效果進行分析,驗證利尿合劑臨床安全性,為臨床治療術后尿潴留探尋安全、有效、易行的方法提供研究依據(jù)。方法:選自2015年6月至2016年2月南京市中醫(yī)院腰麻下混合痔術后患者,選取符合標準的60例尿潴留患者,隨機分為利尿合劑組和哈樂組,各30例。哈樂組為對照組。術后發(fā)生尿潴留患者予以利尿合劑1付。利尿合劑組成:黃芪30g豬苓9g黃柏6g車前草12g烏藥6g甘草梢3g知母9g通草6g陳皮5g桂心(后下)0.6g。煎為250ml,頓服。對每例患者的觀察指標進行統(tǒng)計,治療無效者予以導尿。哈樂組為對照組,術后發(fā)生尿潴留患者予以口服哈樂一粒,0.2mg,對每例患者的觀察指標進行記錄。治療無效者予以導尿。對兩組患者治療后的有效率、首次排出尿液時間、首次尿量以及有無不良反應等評價指標進行記錄和統(tǒng)計學分析。結果:(1)兩組患者在年齡、性別、結扎痔核數(shù)量、病程、手術時間等基本資料方面經(jīng)過統(tǒng)計學對比分析,無差異,具有統(tǒng)計學可比性。(2)利尿合劑組和哈樂組在發(fā)生尿潴留后按照預定的治療方案進行治療和干預后,臨床療效存在顯著差異。利尿合劑組16例治愈,7例好轉(zhuǎn),7例無效導尿,總有效率76.66%;哈樂組5例治愈,7例好轉(zhuǎn),18例無效導尿,總有效率40%。兩組相比P0.05,統(tǒng)計學差異顯著。(3)在首次排尿時間上,利尿合劑組短于哈樂組,兩組相比P0.05,在首次尿量上,利尿合劑組多于哈樂組,兩組相比P0.05.結論:利尿合劑組與哈樂組相比,利尿合劑治療混合痔術后尿潴留的有效率高療效更突出。利尿合劑組用藥后距首次排尿時間間隔短,首次排尿量多,說明利尿合劑較哈樂不僅起效快,而且在臨床效果上更明顯。
[Abstract]:Objective: to analyze the clinical effect of diuretic mixture in the treatment of urinary retention after mixed hemorrhoids, to verify the clinical safety of diuretic mixture, and to provide the basis for the study of safe, effective and feasible methods for the treatment of postoperative urinary retention. Methods: from June 2015 to February 2016, 60 patients with urinary retention in Nanjing Hospital of traditional Chinese Medicine were randomly divided into two groups: diuretic mixture group (n = 30) and halo group (n = 30). Halo group was used as control group. Postoperative patients with urinary retention were given diuretic mixture 1. The composition of the mixture: Astragalus membranaceus 30g, Polyporus umbellata 9g, Radix Phellodendri chinensis 6g, Astragalus chinensis 6g, Radix Glycyrrhiza 6g, Radix Glycyrrhiza 6g. Fried for 250 ml, dressed. The observation indexes of each patient were statistically analyzed, and the patients with ineffective treatment were given urethral catheterization. The control group was used as the control group. The patients with urinary retention were given orally one pill 0.2 mg. The observation indexes of each patient were recorded. Urethral catheterization is given to those who have not been treated. The effective rate, the time of first excretion of urine, the first urine volume and the adverse reaction were recorded and analyzed statistically. Results: (1) there was no significant difference between the two groups in age, sex, number of ligated hemorrhoids, course of disease, operation time and so on. (2) there were significant differences in clinical efficacy between the diuretic mixture group and the halo group after the treatment and intervention according to the scheduled treatment plan after the occurrence of urinary retention. In the diuretic mixture group, 16 cases were cured, 7 cases were improved, 7 cases were ineffective catheterization, the total effective rate was 76.66%, while in the halo group, 5 cases were cured, 7 cases were improved, 18 cases were ineffective catheterization, the total effective rate was 40%. The difference between the two groups was significant (P 0.05). (3) in the first time of urination, the diuretic mixture group was shorter than the halo group, and the two groups compared to the P0.05 group. In the first urine volume, the diuretic mixture group was more than the halo group, and the two groups were compared to the P0.05 group. Conclusion: compared with halo group, the effective rate of diuretic mixture in treating postoperative urinary retention of mixed hemorrhoids is more effective. The interval between the first urination and the first urination time was short and the first urination volume was much in the diuretic mixture group, which indicated that the diuretic mixture not only had a faster effect than Halle, but also had a more obvious clinical effect.
