體外沖擊波對(duì)早中期膝關(guān)節(jié)骨性關(guān)節(jié)炎患者關(guān)節(jié)液中IL-1β、TNF-α及MMP-13表達(dá)的影響

【摘要】:[目的]觀察體外沖擊波對(duì)膝關(guān)節(jié)骨性關(guān)節(jié)炎患者關(guān)節(jié)液中IL-1β、TNF-α及MMP-13表達(dá)的影響,探討其治療膝關(guān)節(jié)骨性關(guān)節(jié)炎的機(jī)制。[方法]將63例膝關(guān)節(jié)骨性關(guān)節(jié)炎患者分為兩組,治療組采用體外沖擊波治療,對(duì)照組采用塞來(lái)昔布膠囊治療,均治療4周,分別在治療前后及治療后6個(gè)月檢測(cè)兩組患者關(guān)節(jié)液中IL-1β、TNF-α及MMP-13的含量。[結(jié)果]治療后,兩組患者關(guān)節(jié)液中IL-1β、TNF-α及MMP-13的含量均明顯低于治療前(P0.05),其中治療組與對(duì)照組含量相當(dāng)(P0.05)。治療后6個(gè)月隨訪,兩組患者關(guān)節(jié)液中IL-1β、TNF-α及MMP-13的含量均較治療后上升(P0.05),但仍明顯低于治療前(P0.05),而治療組的含量明顯低于對(duì)照組(P0.05)。[結(jié)論]體外沖擊波能有效地下調(diào)膝關(guān)節(jié)骨性關(guān)節(jié)炎患者關(guān)節(jié)液中IL-1β、TNF-α及MMP-13的表達(dá),從而抑制炎癥反應(yīng),延緩病程發(fā)展,且與塞來(lái)昔布相比療效更佳。
[Abstract]:[objective] to observe the effect of extracorporeal shock wave on the expression of IL-1 尾, TNF- 偽 and MMP-13 in the articular fluid of patients with knee osteoarthritis, and to explore the mechanism of the treatment of knee osteoarthritis. [methods] Sixty-three patients with knee osteoarthritis were divided into two groups. The treatment group was treated with extracorporeal shock wave and the control group with celecoxib capsule for 4 weeks. The contents of IL-1 尾, TNF- 偽 and MMP-13 in articular fluid were measured before and after treatment. [results] after treatment, the contents of IL-1 尾, TNF- 偽 and MMP-13 in articular fluid of the two groups were significantly lower than those before treatment (P0.05), and the contents of IL-1 尾, TNF- 偽 and MMP-13 in the treated group were similar to those in the control group (P0.05). Six months after treatment, the levels of IL-1 尾, TNF- 偽 and MMP-13 in articular fluid of the two groups were higher than those after treatment (P0.05), but still significantly lower than those before treatment (P0.05). The content of the treatment group was significantly lower than that of the control group (P0.05). [conclusion] Extracorporeal shock wave can effectively down-regulate the expression of IL-1 尾, TNF- 偽 and MMP-13 in the articular fluid of patients with knee osteoarthritis, thus inhibit inflammatory reaction, delay the course of disease, and the curative effect is better than celecoxib.
