【摘要】：目的探討引起燒傷創(chuàng)面病原菌感染發(fā)生的相關(guān)因素及病原菌分布狀況,為燒傷感染的預(yù)防及治療提供參考。方法收集2012年1月-2015年12月在醫(yī)院診治的212例燒傷患者臨床資料,回顧性分析創(chuàng)面病原菌培養(yǎng)結(jié)果及引起感染的相關(guān)因素。結(jié)果 212例患者發(fā)生創(chuàng)面感染者164例,感染率為77.36%;創(chuàng)面分泌物中培養(yǎng)出185株病原菌,其中革蘭陰性菌131株占70.81%,革蘭陽(yáng)性菌49株占26.49%,真菌5株占2.70%,感染病原菌以銅綠假單胞菌、鮑曼復(fù)合醋酸鈣不動(dòng)桿菌、金黃色葡萄球菌為主,分別占18.38%、15.14%、11.89%;單因素及多因素logistic回歸分析顯示,燒傷面積越大、燒傷程度越深、預(yù)防使用抗菌藥物種類(lèi)越多感染率越高(P0.05),入院后預(yù)防使用抗菌藥物、合并休克、平時(shí)有自購(gòu)抗菌藥物濫用習(xí)慣、有長(zhǎng)期服用糖皮質(zhì)激素治療疾病史等因素時(shí)感染率高(P0.05)。結(jié)論燒傷發(fā)生后創(chuàng)面感染率極高,感染病原菌主要是革蘭陰性菌,引起燒傷感染的因素較多,應(yīng)針對(duì)性的進(jìn)行干預(yù)以降低感染發(fā)生。
[Abstract]:Objective to investigate the related factors and distribution of pathogenic bacteria in burn wound, and to provide reference for the prevention and treatment of burn infection. Methods the clinical data of 212 burn patients were collected from January 2012 to December 2015. The culturing results of pathogenic bacteria and the related factors of infection were analyzed retrospectively. Results 164 cases of wound infection occurred in 212 patients, the infection rate was 77.36; 185 strains of pathogenic bacteria were cultured in wound secretion, of which 131 strains were Gram-negative bacteria (70.81strains), 49 Gram-positive bacteria (49 strains) and 5 fungi strains (2.70%). The pathogens were infected by Pseudomonas aeruginosa. Baumann compound Acinetobacter calcium Acinetobacter, Staphylococcus aureus, accounted for 18.38% 15.14% 11.89%; Univariate and multivariate logistic regression analysis showed that the larger the burn area, the deeper the burn degree, the higher the infection rate of prophylactic use of antimicrobial agents (P0.05). Usually have the habit of self-purchase antimicrobial abuse, long-term use of glucocorticoid treatment of disease history and other factors when the infection rate was high (P0.05). Conclusion the infection rate of burn wound is very high, the main pathogenic bacteria are Gram-negative bacteria, there are many factors causing burn infection, so intervention should be carried out in order to reduce the incidence of infection.
