【摘要】：目的觀察負(fù)壓封閉持續(xù)引流聯(lián)合常規(guī)清創(chuàng)及帶蒂骨膜瓣封閉額竇腔的方法治療開顱術(shù)后額竇炎的臨床療效。方法回顧性分析該院2006年1月至2014年12月確診的31例開顱術(shù)后出現(xiàn)額竇炎患者的臨床資料。17例采用單純清創(chuàng)引流治療,14例采用負(fù)壓封閉持續(xù)引流聯(lián)合常規(guī)清創(chuàng)治療,術(shù)后隨訪1年以上。結(jié)果單純清創(chuàng)引流治療治愈12例,5例術(shù)后復(fù)發(fā),治愈率為70.6%,負(fù)壓封閉持續(xù)引流聯(lián)合常規(guī)清創(chuàng)引流治療治愈14例,術(shù)后無復(fù)發(fā),治愈率為100.0%。負(fù)壓封閉持續(xù)引流聯(lián)合常規(guī)清創(chuàng)治療治愈率高于單純清創(chuàng)引流治療(P0.05)。結(jié)論采用負(fù)壓封閉引流聯(lián)合常規(guī)清創(chuàng)及帶蒂骨膜瓣封閉額竇腔的方法,能夠促進(jìn)感染灶清除及創(chuàng)面愈合,明顯提高治愈率。
[Abstract]:Objective to observe the clinical effect of negative pressure continuous drainage combined with conventional debridement and pedicle periosteum flap sealing frontal sinus cavity in the treatment of frontal sinusitis after craniotomy. Methods the clinical data of 31 patients with frontal sinusitis after craniotomy in our hospital from January 2006 to December 2014 were retrospectively analyzed. 17 cases were treated with debridement and drainage, 14 cases were treated with negative pressure sealing and continuous drainage combined with conventional debridement. All patients were followed up for more than 1 year. Results 12 cases were cured by simple debridement and drainage, 5 cases recurred after operation, the cure rate was 70.6%, 14 cases were cured by negative pressure continuous drainage combined with conventional debridement and drainage, no recurrence was found after operation, and the cure rate was 100.0%. The cure rate of negative pressure continuous drainage combined with conventional debridement was higher than that of pure debridement and drainage (P0.05). Conclusion negative pressure sealing and drainage combined with conventional debridement and pedicle periosteum flap to seal frontal sinus cavity can promote the clearance of infected foci and wound healing and improve the cure rate.
