【摘要】：目的評價終末期腎病患者采用人工血管建立動靜脈內(nèi)瘺(AVG)的長期通暢率、術(shù)后并發(fā)癥及處理。方法對2012年4月至2016年7月在本中心行AVG術(shù)的155例患者的病例資料進行回顧性分析。觀察術(shù)后并發(fā)癥的情況和統(tǒng)計術(shù)后初級通暢率、輔助初級通暢率及次級通暢率。結(jié)果本組患者共建立158例次AVG,技術(shù)成功率為100%。圍術(shù)期死亡率為0,平均隨訪時間18.9(3~56)個月。初級通暢期平均13.6(1~37)個月,1年初級通暢率為72.13%,1年輔助初級通暢率為83.61%,1年次級通暢率為98.36%。2年初級通暢率為43.10%,2年輔助初級通暢率為44.83%,2年次級通暢率為93.10%。隨訪期間內(nèi),149例患者術(shù)后出現(xiàn)不同程度的肢體腫脹,3.2周左右消退。55例患者發(fā)生內(nèi)瘺狹窄(121例次),38例患者血栓形成(61例次)。38例患者和15例患者分別出現(xiàn)Ⅰ期和Ⅱ期竊血綜合征。假性動脈瘤4例,感染2例。結(jié)論 AVG術(shù)后并發(fā)癥整體可控,經(jīng)過積極處理也具有較高的次級通暢率,是無法建立AVF時重要的替代選擇。但是AVG長期初級通暢率目前并不高,隨訪維護成本較高,AVG術(shù)前應(yīng)對患者和家屬進行充分告知,使其對AVG有合理的預(yù)期。
[Abstract]:Objective to evaluate the long-term patency rate, postoperative complications and management of arteriovenous fistula (AVG) in patients with end-stage nephropathy. Methods the data of 155 patients undergoing AVG from April 2012 to July 2016 were retrospectively analyzed. The primary patency rate, secondary patency rate and secondary patency rate were observed. Results the success rate of AVG, technique was 100 times. The perioperative mortality was 0 and the mean follow-up time was 18.9 (3 / 56) months.
