【摘要】：目的比較雙帶絆紐扣鋼板和金屬螺釘固定治療下脛腓骨聯(lián)合損傷的臨床療效。方法將45例踝部骨折伴下脛腓聯(lián)合損傷患者按入院日期奇偶數(shù)分成兩組:A組(25例)采用雙帶絆紐扣鋼板固定治療,B組(20例)采用金屬螺釘固定治療(10~12周取出金屬螺釘)。術后6個月對兩組踝關節(jié)AOFAS功能評分、踝關節(jié)活動度進行臨床評估,測量脛骨遠端后外側緣與腓骨遠端內(nèi)側緣之間的距離(TFCS)和脛骨前結節(jié)外側緣與腓骨內(nèi)側緣之間水平距離(TFO),進行影像學評價。結果患者均獲得隨訪,時間6~15個月。術后6個月兩組的踝關節(jié)AOFAS功能評分、踝關節(jié)活動度、TFCS和TFO比較差異均無統(tǒng)計學意義(P0.05)。結論雙帶絆紐扣鋼板治療下脛腓聯(lián)合損傷可達到與金屬螺釘固定相同的效果,并且能保留生理性微動,有利于早期負重活動,降低了內(nèi)固定物松動或斷裂的發(fā)生率,更利于關節(jié)功能康復。
[Abstract]:Objective to compare the clinical effect of double-band trip button plate and metal screw fixation in the treatment of combined injury of lower tibia and fibula. Methods Forty-five patients with ankle fracture associated with injury of lower tibiofibular syndesmosis were divided into two groups: group A (25 cases) were treated with double-band trip-button plate fixation. Group B (20 cases) were treated with metal screw fixation (10 ~ 12 weeks). At 6 months after operation, the AOFAS score and the range of motion of ankle joint in both groups were evaluated. The distance between the posterolateral margin of the distal tibia and the medial margin of the fibula and the horizontal distance between the lateral margin of the anterior tubercle of the tibia and the medial margin of the fibula were measured by (TFCS) for imaging evaluation. Results all patients were followed up for 6 ~ 15 months. Six months after operation, there was no significant difference in ankle AOFAS function score, ankle motion, TFCS and TFO between the two groups (P0.05). Conclusion Dual-band trip-button plate can treat the injury of the tibiofibular syndesmosis with the same effect as the metal screw fixation, and it can preserve the physiological fretting, which is beneficial to the early weight-bearing activity and reduces the incidence of looseness or fracture of the internal fixator. More conducive to the rehabilitation of joint function.
