【摘要】：目的探討微創(chuàng)穿刺輔助開顱減壓搶救高血壓腦出血致腦疝患者的意義。方法 92例高血壓病腦出血患者納入研究,所有患者治療方案隨機(jī)分為3組,血腫微創(chuàng)穿刺引流組采取顱內(nèi)血腫微創(chuàng)穿刺輔助開顱血腫清除手術(shù),腦室微創(chuàng)穿刺組采取腦室微創(chuàng)穿刺輔助開顱血腫清除手術(shù),傳統(tǒng)甘露醇組采取甘露醇輔助傳統(tǒng)開顱血腫清除手術(shù)。分析3組患者預(yù)后及操作相關(guān)指標(biāo)。結(jié)果腦室微創(chuàng)穿刺組瞳孔回復(fù)時間及腦疝持續(xù)時間最短并獲得最少的病死率(P0.01),在操作耗時及非死亡率方面,血腫微創(chuàng)穿刺引流組與腦室微創(chuàng)穿刺組等同;血腫微創(chuàng)穿刺引流組瞳孔回復(fù)時間、腦疝持續(xù)時間及病死率均優(yōu)于傳統(tǒng)甘露醇組(P0.05)。結(jié)論微創(chuàng)穿刺血腫引流與腦室微創(chuàng)穿刺等微創(chuàng)操作較傳統(tǒng)甘露醇輔助開顱血腫清除術(shù)能更快解除腦疝壓迫,從而改善患者預(yù)后。
[Abstract]:Objective to explore the significance of minimally invasive puncture assisted craniotomy in the treatment of cerebral hernia caused by hypertensive intracerebral hemorrhage. Methods 92 patients with hypertensive intracerebral hemorrhage were included in the study. All the patients were randomly divided into 3 groups. The patients in the hematoma minimally invasive puncture drainage group were treated with intracranial hematoma minimally invasive puncture assisted craniotomy and hematoma clearance. The ventricular minimally invasive puncture group was used to assist the craniotomy hematoma removal, and the traditional mannitol group was used to assist the traditional craniotomy hematoma removal operation. The prognosis and operation related indexes were analyzed in 3 groups. Results the recovery time of pupil and the duration of cerebral hernia were the shortest and the least mortality was obtained in the group of minimally invasive puncture of cerebral ventricle (P0.01). In terms of operation time and non-mortality, the group of hematoma minimally invasive puncture and drainage was the same as the group of minimally invasive puncture of cerebral ventricle. The recovery time of pupil, the duration of cerebral hernia and the mortality of hematoma minimally invasive puncture drainage group were better than those of traditional mannitol group (P0.05). Conclusion minimally invasive procedures such as minimally invasive hematoma drainage and minimally invasive intracerebroventricular puncture are more effective than traditional mannitol assisted craniotomy to relieve the compression of cerebral hernia and improve the prognosis of the patients.
