【摘要】：目的為了創(chuàng)新手術(shù)部位感染監(jiān)管方法,遴選適合公開的手術(shù)部位感染指標(biāo),為實(shí)施手術(shù)部位感染率的公開提供參考。方法建立由17~20名國內(nèi)權(quán)威專家組成的德爾菲法專家組,通過文獻(xiàn)法確定供篩選的手術(shù)部位感染指標(biāo),專題小組討論法確定指標(biāo)的選擇標(biāo)準(zhǔn);采用Likert 5分制法,由專家依據(jù)選擇標(biāo)準(zhǔn)對指標(biāo)進(jìn)行評分;經(jīng)過兩輪專家咨詢,用界值法篩選指標(biāo),確定適合公開的手術(shù)部位感染指標(biāo)集。結(jié)果經(jīng)過二輪篩選共納入包括剖宮產(chǎn)手術(shù)部位感染率、疝縫術(shù)手術(shù)部位感染率、乳腺切除術(shù)手術(shù)部位感染率等13個(gè)適合公開的手術(shù)部位感染率指標(biāo),并且9個(gè)選擇標(biāo)準(zhǔn)的平均分均3.5,且變異系數(shù)0.3,總評分在35.50~37.40;其中,9個(gè)指標(biāo)為特定手術(shù)的手術(shù)部位感染率,占69.2%。結(jié)論初步遴選出適合公開的手術(shù)部位感染指標(biāo)集,為實(shí)施透明監(jiān)管這種新的監(jiān)管方式打下基礎(chǔ)。為了提高可比性,手術(shù)部位感染指標(biāo)在公開前還需要進(jìn)行風(fēng)險(xiǎn)調(diào)整。
[Abstract]:Objective in order to innovate the monitoring method of surgical site infection, select the suitable surgical site infection index, and provide reference for the open operation site infection rate. Methods the expert group of Delphi method was established, which was composed of 1720 experts from China. The infection index of surgical site for screening was determined by literature method, and the selection criteria of index were determined by panel discussion. The index was scored by experts according to the selection criteria and the index set suitable for open operation site was determined through two rounds of expert consultation and the selection of indicators by the method of boundary value. Results the infection rate of cesarean section site, hernia suture operation site and mammary resection site infection rate were all included after two rounds of screening. The average score of the nine selection criteria was 3.5, and the coefficient of variation was 0.3, the total score was 35.50 ~ 37.40; Among them, 9 indexes were specific surgical site infection rate (69.2%). Conclusion A suitable index set of surgical site infection was preliminarily selected to lay the foundation for the implementation of transparent supervision. In order to improve comparability, surgical site infection indicators need to be adjusted before they are made public.
