異種凍干骨復(fù)合自體骨髓行兔脊柱融合術(shù)的實(shí)驗(yàn)研究

【摘要】:目的:采用復(fù)合自體骨髓的異種凍干骨行兔腰椎橫突間植骨融合手術(shù),從影像學(xué)、組織學(xué)等方面檢測(cè)融合效果,從而提供一種效果優(yōu)良、來源充足的脊柱植骨融合材料。方法:將36只兔子隨機(jī)平均分為3組,A組(12只):異種凍干骨混合自身骨髓,B組(12只):單純異種凍干骨,C組(12只):自體髂骨,將植骨材料植入各自組別的模型橫突間。術(shù)后12周處死模型,收集并處理好標(biāo)本,分別采用大體觀察、影像學(xué)、組織學(xué)、生物力學(xué)檢測(cè)等觀察、分析融合成效。結(jié)果:術(shù)后12周,從大體觀察、手動(dòng)檢查上比較,A、C兩組的融合率高于B組且有統(tǒng)計(jì)學(xué)差異(p0.05)。影像學(xué)檢查根據(jù)Curylo分級(jí)評(píng)分A、C兩組的融合率高于B組,有顯著性差異。生物力學(xué)檢測(cè)植骨區(qū)標(biāo)本承受三點(diǎn)彎曲載荷時(shí),單純異種凍干骨組的最大彎曲三點(diǎn)破壞平均載荷為184.32±31.07N,自體髂骨移植組的最大彎曲三點(diǎn)破壞平均載荷為304.29±18.36N,異種凍干骨復(fù)合自體骨髓組的最大彎曲三點(diǎn)破壞平均載荷為252.47±25.45N,B組的抗彎曲硬度明顯低于A、C兩組,統(tǒng)計(jì)學(xué)分析有差異(p0.05),而A組與C組在統(tǒng)計(jì)學(xué)上無差異(p0.05)。組織學(xué)上,A組可見大量成骨細(xì)胞生成,植骨材料內(nèi)可見成熟的骨細(xì)胞;B組只有少量成骨細(xì)胞形成;C組可見新生的骨小梁形成。結(jié)論:復(fù)合自體骨髓的異種凍干骨可有效的提高脊柱融合效果,與自體髂骨移植無明顯差異,可以作為脊柱融合手術(shù)的植骨材料選擇之一。
[Abstract]:Objective: to evaluate the fusion effect of intertransverse bone graft of lumbar vertebrae in rabbits with allogeneic freeze-dried bone marrow combined with autologous bone marrow, and to provide a good and abundant material for spinal graft fusion. Methods: Thirty-six rabbits were randomly divided into three groups: group A (n = 12), group B (n = 12), group C (n = 12): autogenous iliac bone, group B (n = 12): allogeneic freeze-dried bone, group C (n = 12): autogenous iliac bone, group B (n = 12). Bone graft materials were implanted into the transverse process of each group. 12 weeks after operation, the models were sacrificed and the specimens were collected and processed. The fusion effect was analyzed by observing gross observation, imaging, histology, biomechanical examination and so on. Results: at 12 weeks after operation, the fusion rate of group A and C was higher than that of group B by gross observation and manual examination (p0.05). The fusion rate of group A and C was higher than that of group B according to Curylo score, and there was a significant difference between the two groups. Biomechanical analysis showed that the mean load of maximum bending three point failure was 184.32 鹵31.07Nin the xenogeneic freeze-dried bone group and 304.29 鹵18.36Nin the autogenous iliac bone graft group. The mean bending hardness of group B was significantly lower than that of group A (252.47 鹵25.45 NM), which was significantly lower than that of group A (p 0.05), and the mean load of maximum bending three-point failure in group B was significantly lower than that in group A (P < 0.05). There was no statistical difference between group A and group C (p0.05). Histologically, a large number of osteoblasts were found in group A, mature bone cells were found in bone graft materials, only a few osteoblasts were formed in group B, and new trabeculae formation was observed in group C. Conclusion: allogeneic freeze-dried bone combined with autologous bone marrow can effectively improve the effect of spinal fusion, and there is no significant difference with autogenous iliac bone graft. It can be used as a choice of bone graft materials for spinal fusion surgery.
