【摘要】：目的探討瞼裂不對(duì)稱單瞼的重瞼成形術(shù)手術(shù)方法。方法 26例瞼裂不對(duì)稱的重瞼手術(shù)者,術(shù)前坐位平視自然睜眼狀態(tài)下雙眼瞼裂寬度相差1.0~2.0(1.42±0.20)mm。按照測(cè)量數(shù)據(jù)將26例患者分為3種類型:(1)上瞼皮膚松垂量不等型;(2)瞼裂寬度不等型;(3)上瞼皮膚松垂量不等合并瞼裂寬度不等型。根據(jù)患者雙眼瞼部特點(diǎn),采用差量去除瞼緣松垂皮膚、提上瞼肌前徙術(shù)或兩種手術(shù)方法并用的方案,行重瞼成形術(shù)進(jìn)行矯正。術(shù)后6~12個(gè)月隨訪時(shí)測(cè)量雙眼瞼裂寬度差值,記錄末次差值,對(duì)數(shù)據(jù)行配對(duì)樣本t檢驗(yàn)。結(jié)果術(shù)后7例患者出現(xiàn)行提上瞼肌前徙術(shù)側(cè)輕度瞼裂閉合不全情況,予以紅霉素眼膏治療,未出現(xiàn)暴露性角膜炎等并發(fā)癥,于術(shù)后1周到1個(gè)月閉合。隨訪6~12個(gè)月,9例單純行差量去皮重瞼術(shù)的患者,術(shù)后2例半年后復(fù)發(fā)。其余患者重瞼線條流暢,雙眼瞼裂無(wú)明顯不對(duì)稱,不對(duì)稱瞼裂未見(jiàn)復(fù)發(fā)。末次隨訪時(shí)雙眼瞼裂寬度相差0.1~0.5(0.21±0.08)mm,與術(shù)前比較,差值明顯縮小(t=38.912,P0.01)。結(jié)論差量去皮或合并提上瞼肌前徙術(shù)的重瞼成形術(shù),對(duì)瞼裂不對(duì)稱單瞼矯正效果良好。
[Abstract]:Objective to investigate the double blepharoplasty of asymmetric single eyelid with palpebral fissure. Methods 26 cases of double eyelid operation with asymmetric palpebral fissure were performed. The difference in the width of eyelid fissure between the two eyes was 1.0 and 2.0 (1. 42 鹵0. 20) mm.. According to the measured data, 26 patients were divided into three types: (1) the upper eyelid skin sagging unequal type; (2) the eyelid fissure width unequal type; (3) the upper eyelid skin sagging volume unequal combined with eyelid fissure width unequal type. According to the characteristics of double eyelids, double eyelid plasty was carried out by using the method of removing palpebral margin loose skin, levator palpebral muscle recession or two kinds of operation methods. The difference of eyelid fissure width was measured and the last difference was recorded at the follow-up of 6 ~ 12 months after operation. Results there were 7 patients with mild eyelid fissure closure after levator palpebral muscle recession. Erythromycin ophthalmic ointment was given without any complications such as exposed keratitis, which was closed 1 week to 1 month after operation. Follow up for 6 ~ 12 months, 9 cases of simple differential volume removal of heavy eyelid operation, 2 cases of recurrence half a year after operation. The other patients had smooth double eyelid lines, no obvious asymmetry of eyelid fissure, and no recurrence of asymmetric eyelid fissure. At the last follow-up, the difference in the width of eyelid fissure was 0. 1 鹵0. 5 (0. 21 鹵0. 08) mm, compared with that before operation. The difference was significantly reduced (t = 38. 912, P 0. 01). Conclusion double blepharoplasty with differential exfoliation or combined with levator blepharoplasty has a good effect on the correction of asymmetrical eyelid cleft eyelid.
