【摘要】：目的:探討在內(nèi)側(cè)撐開脛骨高位截骨矯正膝內(nèi)翻術(shù)后脛骨平臺后傾角改變的影響因素。方法:通過對CT數(shù)據(jù)進(jìn)行三維重建建立脛骨近端模型,并以此模擬內(nèi)側(cè)撐開脛骨高位截骨術(shù),分析截骨矯正過程中合頁軸的方向及內(nèi)翻矯正程度與術(shù)后脛骨平臺后傾角變化之間的關(guān)系。結(jié)果:與標(biāo)準(zhǔn)矢狀位前后方向相比,合頁軸的方向指向前外側(cè)方向會導(dǎo)致術(shù)后脛骨平臺后傾角增加;同時較大內(nèi)翻矯正角度也導(dǎo)致術(shù)后脛骨平臺后傾角增加較多。結(jié)論:內(nèi)側(cè)撐開脛骨高位截骨術(shù)后脛骨平臺后傾角的變化與截骨矯正過程中合頁軸的方向及內(nèi)翻矯正程度密切相關(guān)。
[Abstract]:Objective: to investigate the influencing factors of tibial plateau inclination after high tibial osteotomy with medial distraction. Methods: the proximal tibia model was established by 3D reconstruction of CT data, and the high tibial osteotomy was simulated by medial distraction. The relationship between the direction of the hinge axis and the degree of varus correction and the posterior inclination of tibial plateau during osteotomy was analyzed. Results: compared with the anteroposterior direction of the standard sagittal position, the direction of the hinge axis pointing to the anterolateral direction would lead to the increase of the posterior inclination angle of the tibial plateau after operation, and the larger angle of the varus correction would also lead to the increase of the posterior inclination angle of the tibial plateau. Conclusion: the change of posterior tilt angle of tibial plateau after high tibial osteotomy with medial distraction is closely related to the direction of hinge axis and the degree of varus correction during osteotomy.
