基于活動平板訓(xùn)練的虛擬現(xiàn)實技術(shù)在全膝關(guān)節(jié)表面置換術(shù)患者中的應(yīng)用

【摘要】:目的探討基于活動平板訓(xùn)練的虛擬現(xiàn)實技術(shù)(Virtual reality,VR)在全膝關(guān)節(jié)表面置換術(shù)(Total knee arthroplasty,TKA)患者中的應(yīng)用。方法將我院關(guān)節(jié)外科行單側(cè)TKA術(shù)后的62例患者隨機均分為觀察組(31例)和對照組(31例)。兩組患者術(shù)后均接受常規(guī)的康復(fù)訓(xùn)練,觀察組在此基礎(chǔ)上進行基于活動平板訓(xùn)練的VR訓(xùn)練。對比兩組患者術(shù)后VAS疼痛評分、膝關(guān)節(jié)腫脹程度及主動活動度、美國特種外科醫(yī)院膝關(guān)節(jié)功能評分(HSS)以及臨床療效。結(jié)果兩組術(shù)后3d、5d、7d VAS評分及術(shù)后5d、7d膝關(guān)節(jié)腫脹度差異具有統(tǒng)計學(xué)意義(P0.05);但術(shù)后第3d兩組膝關(guān)節(jié)腫脹度差異無統(tǒng)計學(xué)意義(P0.05);觀察組術(shù)后2周、6周膝關(guān)節(jié)主動活動度顯著高于對照組(P0.05),兩組術(shù)前及術(shù)后3個月、6個月膝關(guān)節(jié)主動活動度差異無統(tǒng)計學(xué)意義(P0.05)。兩組患者術(shù)前HSS評分差異無統(tǒng)計學(xué)意義(P0.05),術(shù)后1周、2周、6周及3個月兩組差異具有統(tǒng)計學(xué)意義(P0.05)。觀察組患者優(yōu)良率明顯高于對照組,差異具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論基于活動平板訓(xùn)練的VR技術(shù)可以有效的緩解TKA術(shù)后早期康復(fù)的疼痛、減輕膝關(guān)節(jié)的腫脹,提高術(shù)后膝關(guān)節(jié)的功能。
[Abstract]:Objective to investigate the application of virtual reality technique (Virtual reality,VR) based on treadmill training in patients with total knee joint surface replacement (Total knee arthroplasty,TKA). Methods Sixty-two patients undergoing unilateral TKA surgery in our hospital were randomly divided into observation group (n = 31) and control group (n = 31). The patients in both groups received routine rehabilitation training, and the observation group received VR training based on treadmill training. VAS pain score, knee joint swelling degree and active activity, knee joint function score (HSS) and clinical efficacy were compared between the two groups. Results there was significant difference in VAS score between the two groups on the 3rd and 5th day after operation (P0.05), but there was no significant difference in the knee joint swelling between the two groups on the 3rd day after operation (P0.05). The knee active motion in the observation group was significantly higher than that in the control group at 2 and 6 weeks after operation (P0.05). There was no significant difference between the two groups before and after 3 months and 6 months of knee joint active motion (P0.05). There was no significant difference in preoperative HSS score between the two groups (P0.05), but there was significant difference between the two groups at 1 week, 2 weeks, 6 weeks and 3 months after operation (P0.05). The excellent and good rate of the observation group was significantly higher than that of the control group, the difference was statistically significant (P0.05). Conclusion the VR technique based on treadmill training can effectively relieve the pain of early recovery after TKA, alleviate the swelling of knee joint, and improve the function of knee joint after TKA.
