【摘要】：目的對(duì)比觀察腔鏡下與開(kāi)放性甲狀腺手術(shù)的臨床療效。方法將80例甲狀腺疾病患者隨機(jī)分為兩組各40例,觀察組行腔鏡下甲狀腺手術(shù),對(duì)照組行開(kāi)放性甲狀腺手術(shù),觀察兩組圍手術(shù)期指標(biāo)、術(shù)后疼痛及并發(fā)癥情況。結(jié)果兩組手術(shù)時(shí)間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);與對(duì)照組比較,觀察組住院時(shí)間、拔管時(shí)間縮短,術(shù)中出血量、引流總量、術(shù)后VAS減少(P均0.05);觀察組術(shù)后出現(xiàn)出血、喉返神經(jīng)損傷及甲狀旁腺功能低下各1例,對(duì)照組術(shù)后出現(xiàn)出血2例、窒息3例、喉返神經(jīng)損傷2例、甲狀旁腺功能低下1例;觀察組并發(fā)癥發(fā)生率(7.50%)低于對(duì)照組(20.00%),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論與開(kāi)放性甲狀腺手術(shù)比較,腔鏡下甲狀腺手術(shù)治療能取得較好臨床療效,且手術(shù)損傷小、術(shù)后痛疼減輕、并發(fā)癥少。
[Abstract]:Objective to observe the clinical effect of endoscopic and open thyroid surgery. Methods Eighty patients with thyroid diseases were randomly divided into two groups: 40 patients in each group. The patients in the observation group underwent endoscopic thyroid surgery and the control group underwent open thyroid surgery. The perioperative indexes, postoperative pain and complications were observed in the two groups. Results there was no significant difference in operation time between the two groups (P0.05), and compared with the control group, the hospitalization time, extubation time, intraoperative bleeding volume, total drainage volume and postoperative VAS decreased in the observation group (P 0.05). Bleeding, recurrent laryngeal nerve injury and hypoparathyroidism occurred in 1 case in the observation group, 2 cases in the control group, 3 cases in asphyxia, 2 cases in the injury of the recurrent laryngeal nerve and 1 case in the hypoparathyroidism in the control group. The incidence of complications in the observation group (7.50%) was significantly lower than that in the control group (20.00%) (P0.05). Conclusion compared with open thyroid surgery, endoscopic thyroidectomy can achieve better clinical effect, with less injury, less postoperative pain and less complications.
