【摘要】：目的探討預(yù)防兔肉膜和深筋膜植入擴(kuò)張器移位的方法。方法普通級(jí)實(shí)驗(yàn)家兔40只,隨機(jī)分為兩組,即實(shí)驗(yàn)組20只和對(duì)照組20只。在其一側(cè)背部皮膚肉膜深面自切口潛行分離形成植入腔穴,在腔穴周邊,用3-0絲線間斷縫合肉膜、深筋膜,植入50 m L腎形硅橡膠軟組織擴(kuò)張器。術(shù)后1周,用文刺機(jī)在擴(kuò)張器周圍四點(diǎn)做黑色文刺,用來標(biāo)記擴(kuò)張器位置。實(shí)驗(yàn)組術(shù)后2個(gè)月開始注水直至達(dá)到總量140 m L;對(duì)照組術(shù)后1周開始注水直至達(dá)到總量140 m L。分別觀察和記錄實(shí)驗(yàn)組和對(duì)照組擴(kuò)張器在注水期間是否移位、切口是否裂開、注射壺是否翻轉(zhuǎn)等并發(fā)癥,以及動(dòng)物的死亡率。結(jié)果實(shí)驗(yàn)組中沒有兔子發(fā)生擴(kuò)張器移位,對(duì)照組存活動(dòng)物中有4只兔子發(fā)生擴(kuò)張器移位,兩組擴(kuò)張器移位相比較,差異具有統(tǒng)計(jì)學(xué)意義P0.05㖞;實(shí)驗(yàn)組兔子擴(kuò)張期切口裂開發(fā)生率雖然低于對(duì)照組,但兩組比較沒有統(tǒng)計(jì)學(xué)意義P0.05㖞;同時(shí)兩組擴(kuò)張器在注水壺翻轉(zhuǎn)率、感染率及動(dòng)物死亡率方面相比較都無統(tǒng)計(jì)學(xué)意義P0.05㖞。結(jié)論分離腔隙內(nèi)固定加上延遲擴(kuò)張的手術(shù)方法可有效預(yù)防兔肉膜與深筋膜之間的擴(kuò)張器移位,實(shí)現(xiàn)有效擴(kuò)張。
[Abstract]:Objective to explore the method of preventing the transposition of rabbit meat membrane and deep fascia implant dilator. Methods 40 rabbits of common grade were randomly divided into two groups: experimental group (n = 20) and control group (n = 20). The skin fleshy membrane on one side of the skin was separated from the deep surface of the skin on one side to form an implant cavity point. Around the cavity point, 50 mL renal silicone rubber soft tissue expander was implanted with 3-0 thread suture, deep fascia and 50 mL renal silicone rubber soft tissue expander. One week after operation, the spurs were used to mark the position of the expander at four points around the expander. Two months after operation, the experimental group began to inject water until the total amount reached 140 mL, while the control group began to inject water until the total amount reached 140 mL 1 week after operation. The complications such as the displacement of the dilator during the injection, the split of the incision, the reversal of the injection pot and the mortality of the animals were observed and recorded respectively in the experimental group and the control group. Results there was no dilator shift in the experimental group and 4 rabbits in the control group. The difference was statistically significant (P0.05); the incidence of incision rupture in the experimental group was lower than that in the control group, but there was no statistical significance between the two groups? P0.05; at the same time, the turn-over rate of the dilator in the two groups was lower than that in the control group. There was no significant difference in infection rate and animal mortality (P 0.05). Conclusion the separation of lacunar fixation and delayed dilatation can effectively prevent the transposition of dilator between rabbit meat and deep fascia and achieve effective dilatation.
