【摘要】：目的:探討鼻唇溝帶蒂皮瓣聯(lián)合缺損局部翻轉(zhuǎn)組織瓣在鼻翼缺損整復(fù)中的臨床應(yīng)用效果。方法:2012--2015年我科收治的6例不同原因所致鼻翼全層缺損畸形的患者,采用缺損局部鼻翼旁或鼻背組織瓣翻轉(zhuǎn)后做襯里,鼻唇溝帶蒂皮瓣覆蓋修復(fù)全層鼻翼缺損。術(shù)后觀察皮瓣血運(yùn)情況及遠(yuǎn)期修復(fù)效果。結(jié)果:本組6例患者的皮瓣全部存活,其中1例患者翻轉(zhuǎn)瘢痕組織瓣遠(yuǎn)端出現(xiàn)少許壞死,經(jīng)局部祛痂、換藥后壞死創(chuàng)面愈合良好。6例患者隨訪1~7個(gè)月,重建鼻翼質(zhì)地、顏色、大小與健側(cè)相近,鼻孔形態(tài)良好,鼻腔無通氣障礙,供區(qū)可見線性疤痕,無明顯皺褶、臃腫等,患者對(duì)總體外觀均滿意。結(jié)論:采用鼻唇溝帶蒂皮瓣聯(lián)合缺損局部翻轉(zhuǎn)組織瓣做鼻腔襯里修復(fù)鼻翼全層缺損,重建鼻翼形態(tài)良好,術(shù)后瘢痕不明顯,是一種較理想的方法。
[Abstract]:Objective: to investigate the clinical effect of nasolabial flap pedicled flap combined with local reversed tissue flap in the repair of nasal alar defect. Methods: from 2012 to 2015, 6 cases of full-thickness alar defects caused by different causes were treated in our department. The defect was repaired with the flap pedicled with nasolabial sulcus pedicle. Postoperative blood circulation and long-term repair effect were observed. Results: the flaps of 6 patients were all alive, one of them showed a little necrosis at the distal end of the flap, and the wound healed well after the change of dressing, and 6 patients were followed up for 1 ~ 7 months to reconstruct the texture and color of the nasal wing. The size was similar to that of the normal side, the nostril shape was good, the nasal cavity had no ventilation disturbance, the donor area had linear scar, no obvious wrinkle, bloated and so on. The patients were satisfied with the overall appearance. Conclusion: the nasolabial flap with pedicled flap combined with the local reversed tissue flap is an ideal method to repair the full-thickness defect of the nasal alar and reconstruct the alar with good shape and the scar is not obvious after operation.
