【摘要】：目的：通過研究1：1骨盆DR片制作方法,使之成為放射科的制片規(guī)范,探討其在人工髖關(guān)節(jié)置換術(shù)前準(zhǔn)備中的指導(dǎo)意義。方法：收集52例于2013年12月至2014年10月在廣西醫(yī)科大學(xué)第四附屬醫(yī)院行初次全髖關(guān)節(jié)置換術(shù)患者的臨床資料。通過術(shù)前以一元人民幣硬幣作為參照物制作1：1大小骨盆平片,術(shù)前應(yīng)用模板測(cè)量并預(yù)測(cè)假體型號(hào),與術(shù)中所用實(shí)際假體大小進(jìn)行比較,分析該制作方法的準(zhǔn)確性。結(jié)果：術(shù)前規(guī)范制作的骨盆平片可用于術(shù)前計(jì)劃,并可較好的預(yù)測(cè)假體型號(hào)：術(shù)前預(yù)測(cè)髖臼假體與實(shí)際所用假體完全符合的有34例(65.4%),相差一個(gè)型號(hào)有15例(28.8%),相差≥2個(gè)型號(hào)3例(5.8%),其符合優(yōu)良率為94.2%；股骨假體與實(shí)際假體完全符合為30例(57.7%),相差1個(gè)型號(hào)18例(34.6%),相差≥2個(gè)型號(hào)號(hào)4例(7.7%),股骨假體的符合優(yōu)良率為92.3%。結(jié)論：通過術(shù)前規(guī)范制作真實(shí)尺寸骨盆平片并進(jìn)行模板測(cè)量可精確的預(yù)測(cè)術(shù)中假體型號(hào),為全髖關(guān)節(jié)置換術(shù)提供參考依據(jù),具有一定的臨床指導(dǎo)意義。
[Abstract]:Objective: to study the method of making 1:1 pelvic DR film and to make it the standard of radiology department, and to explore its guiding significance in the preparation of artificial hip arthroplasty. Methods: the clinical data of 52 patients undergoing initial total hip arthroplasty from December 2013 to October 2014 in the fourth affiliated Hospital of Guangxi Medical University were collected. The 1:1 pelvis plain film was made with one yuan coin as reference before operation. The model of prosthesis was measured and predicted by using template before operation. The accuracy of the method was analyzed by comparing with the actual size of prosthesis used during operation. Results: the pelvis plain film made before operation can be used for preoperative planning. There were 34 cases (65.4%) with a difference of one type (28. 8%) and 3 cases (5. 8%). The excellent and good rate of predicting acetabular prosthesis before operation was 94. 2% (5. 8%). There were 30 cases (57.7%) with complete agreement between the femoral prosthesis and the actual prosthesis, 18 cases (34.6%) were different in one model, 4 cases (7.7%) were different from the actual prosthesis. The excellent and good rate of femoral prosthesis was 92.3%. Conclusion: making real size pelvic plain film and measuring template before operation can accurately predict the type of prosthesis during operation and provide reference for total hip arthroplasty. It has certain clinical significance.
