下肢靜脈壓測(cè)定在下肢深靜脈血栓診治中的臨床應(yīng)用

【摘要】:目的:探討下肢靜脈壓在下肢深靜脈血栓(DVT)診治中的臨床應(yīng)用價(jià)值。方法:2013年7月至2014年6月期間,收集筆者所在醫(yī)院住院并經(jīng)彩色多普勒或下肢深靜脈造影[11]檢查確診的患有單側(cè)下肢DVT患者共230例,按照下列不同分類(lèi)各自抽取30例DVT患者(共90例)作為研究對(duì)象,其中男37例,女53例;年齡為18~84歲,平均年齡59.48歲;按病期分為急性期30例,亞急性期30例,慢性期30例;按病理類(lèi)型分為中央型30例,周?chē)?0例,混合型30例。同時(shí)選取同期無(wú)下肢DVT的志愿者20例作為正常對(duì)照組,其中男9例,女11例,年齡為21~65歲,平均年齡38.7歲。測(cè)定2組納入研究者的雙下肢靜態(tài)靜脈壓(P0)、動(dòng)態(tài)靜脈壓(P00)、大小腿周徑差和壓力降低率(Pd)并進(jìn)行比較分析。結(jié)果:病例組不同病期以及不同病理類(lèi)型的P0、P00和大小腿周徑差均高于正常對(duì)照組(P0.01),而Pd則低于正常對(duì)照組(P0.01)。病例組急性期的P0和P00均高于慢性期(P0.01);中央型的P00高于周?chē)秃突旌闲?P0.01),中央型的Pd則低于混合型(P0.01)。正常對(duì)照組左右下肢的上述3項(xiàng)指標(biāo)的差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);病例組不同病期以及不同病理類(lèi)型者患肢的P0和P00均高于健肢,而Pd則均低于健肢(P0.01)。結(jié)論:下肢靜脈壓的測(cè)定可應(yīng)用于臨床對(duì)早期下肢DVT患者的檢測(cè),可作為臨床對(duì)下肢DVT的治療效果評(píng)估的客觀指標(biāo),是一種簡(jiǎn)單實(shí)用的臨床檢測(cè)方法。

According to the following classification, 30 patients (90 cases) with DVT were selected as the study subjects, including 37 males and 53 females, the average age was 59.48 years, and was divided into acute stage (30 cases), subacute stage (30 cases) and chronic stage (30 cases). There were 30 cases of central type, 30 cases of peripheral type and 30 cases of mixed type according to pathological type. At the same time, 20 volunteers without lower extremity DVT were selected as normal control group, including 9 males and 11 females, aged 21 to 65 years, with an average age of 38.7 years. Static venous pressure (P0), dynamic venous pressure (P00), leg circumference difference and pressure reduction rate (Pd) were measured and compared between the two groups. Results: the P0 P00 and leg circumference difference of different pathological types in the case group were higher than those in the normal control group (P0.01), while the Pd was lower than that in the normal control group (P0.01). P0 and P00 in acute stage were higher than that in chronic stage (P0.01), P00 in central type was higher than that in peripheral type and mixed type (P0.01), and Pd in central type was lower than that in mixed type (P0.01). The P0 and P00 of patients with different disease stages and different pathological types were higher than that of healthy limbs, while Pd was lower than that of healthy limbs (P0.01). Conclusion: the determination of venous pressure of lower extremity can be used to detect early lower extremity DVT patients, and can be used as an objective index to evaluate the therapeutic effect of lower extremity DVT. It is a simple and practical clinical detection method.
