【摘要】：目的:比較連續(xù)腰叢神經(jīng)阻滯(continuous lumbar plexus block,CLPB)聯(lián)合多模式鎮(zhèn)痛與患者靜脈自控鎮(zhèn)痛(patient controlled intravenous analgesia,PCIA)聯(lián)合多模式鎮(zhèn)痛在髖關(guān)節(jié)翻修術(shù)術(shù)后鎮(zhèn)痛中的效果評(píng)價(jià)。方法:68例單側(cè)髖關(guān)節(jié)翻修的患者隨機(jī)平均分成CLPB組與PCIA組。2組患者術(shù)前3 d使用帕瑞昔布鈉鎮(zhèn)痛;CLPB組術(shù)后經(jīng)患側(cè)腰叢神經(jīng)周圍置管處使用羅哌卡因鎮(zhèn)痛,PCIA組術(shù)后經(jīng)靜脈使用曲馬多加氟比洛芬酯鎮(zhèn)痛,2組患者持續(xù)鎮(zhèn)痛3 d,且術(shù)后加用帕瑞昔布鈉鎮(zhèn)痛直至運(yùn)動(dòng)時(shí)VAS評(píng)分3分。記錄患者術(shù)后在靜息及運(yùn)動(dòng)狀態(tài)下的視覺(jué)模擬疼痛評(píng)分(visual analogue scale,VAS)、靜脈注射帕瑞昔布鈉用量、鎮(zhèn)痛泵按壓次數(shù)及不良反應(yīng)。結(jié)果:CLPB組患者術(shù)后各時(shí)刻點(diǎn)在靜息狀態(tài)及運(yùn)動(dòng)狀態(tài)下的VAS評(píng)分明顯低于PCIA組(P0.05);CLPB組術(shù)后6、12、24 h在運(yùn)動(dòng)狀態(tài)下的VAS評(píng)分分別為(2.50±0.71)、(3.59±0.99)、(3.67±1.01)分,明顯低于對(duì)應(yīng)時(shí)刻點(diǎn)的PCIA組:(4.97±0.94)分(P=0.00)、(4.85±0.86)分(P=0.00)、(4.32±0.73)分(P=0.00);鎮(zhèn)痛泵按壓次數(shù)及術(shù)后帕瑞昔布鈉用量明顯少于PCIA組(P0.05);不良反應(yīng)發(fā)生率低于PCIA組。結(jié)論:CLPB聯(lián)合多模式鎮(zhèn)痛能有效緩解髖關(guān)節(jié)翻修術(shù)術(shù)后的早期疼痛,其術(shù)后鎮(zhèn)痛效果優(yōu)于PCIA,不良反應(yīng)發(fā)生低,是髖關(guān)節(jié)翻修術(shù)較為理想的鎮(zhèn)痛方案。
[Abstract]:Objective: to evaluate the effect of continuous lumbar plexus block (continuous lumbar plexus block,CLPB) combined with multi-mode analgesia (patient controlled intravenous analgesia,PCIA) and patient-controlled intravenous analgesia (patient controlled intravenous analgesia,PCIA) on postoperative analgesia after hip revision surgery. Methods Sixty-eight patients with unilateral hip revision were randomly divided into two groups: CLPB group and PCIA group. 2 patients were treated with paroxibum sodium for analgesia 3 days before operation. Tramadol plus flurbiprofen was administered intravenously for 3 days, and the postoperative analgesia was performed with paroxib sodium for 3 days, and the VAS score was 3 points at the time of exercise. The visual analogue pain score (visual analogue scale,VAS), the dosage of paroxib sodium intravenously, the times of analgesic pump compression and adverse reactions were recorded. Results the VAS scores in rest and exercise were significantly lower in the control group than in the PCIA group (P 0.05). The VAS scores in the exercise state were (2.50 鹵0.71), (3.59 鹵0.99), (3.67 鹵1.01, respectively. The PCIA group was significantly lower than the corresponding time point group: (4.97 鹵0.94), (P0. 00), (4. 85 鹵0. 86) (P0. 00), (4. 32 鹵0. 73) (P0. 00), the number of analgesic pump compressions and the amount of paroxib sodium after operation were significantly lower than those in the PCIA group (P0.05), and the incidence of adverse reactions was lower than that in the PCIA group (P < 0. 05). Conclusion the effect of PCIA, combined with the multimode analgesia is better than that of PCIA,. It is an ideal analgesic scheme for revision of hip joint.
