【摘要】：目的:面肌痙攣的病因系面神經(jīng)出腦干段(root exit zone,REZ)受血管壓迫,但具體發(fā)病機(jī)制仍不清。我們前期研究證實(shí)責(zé)任血管壁上交感神經(jīng)在該病的產(chǎn)生中起重要作用。本研究將探討交感神經(jīng)末梢釋放的去甲腎上腺素(norepinephrine,NE)及受體在面肌痙攣發(fā)病中的作用。方法:使用Moller法建立面肌痙攣(hemifacial spasm,HFS)SD大鼠動(dòng)物模型,以監(jiān)測(cè)到穩(wěn)定的異常肌反應(yīng)波(abnormal muscle response,AMR)為模型成功的標(biāo)準(zhǔn),隨后對(duì)實(shí)驗(yàn)動(dòng)物的面神經(jīng)行NE受體免疫組化染色,統(tǒng)計(jì)分析AMR波與NE受體表達(dá)量之間的相關(guān)性。接著以相同方法建立面神經(jīng)損傷模型,將面神經(jīng)標(biāo)本用透射電鏡觀察神經(jīng)外膜和Western-Blot檢測(cè)NE受體蛋白,分析神經(jīng)外膜脫髓鞘病理改變與NE受體蛋白表達(dá)量之間的相關(guān)性。結(jié)果:面肌痙攣模型建立后,10只大鼠中有6只出現(xiàn)AMR波,免疫組化結(jié)果提示這6只大鼠面神經(jīng)上NE受體均有異位高表達(dá),而其余4只未監(jiān)測(cè)到AMR波形的大鼠中只有1只NE受體異位高表達(dá)(P0.05)。在神經(jīng)損傷模型,10只大鼠中有7只面神經(jīng)外膜出現(xiàn)脫髓鞘改變,Western-Blot結(jié)果提示這7只大鼠中有6只NE受體蛋白表達(dá)量增高,而其余3只面神經(jīng)外膜未損傷的大鼠NE受體蛋白表達(dá)量無(wú)明顯變化(P0.05,OR=30.33)。結(jié)論:面神經(jīng)外膜破損后可出現(xiàn)跨膜NE受體,該受體在面肌痙攣發(fā)病中起重要作用。
[Abstract]:Objective: the etiology of hemifacial spasm is facial nerve brain stem (root exit zone,REZ) compression by blood vessels, but the pathogenesis is still unclear. Our previous studies have confirmed that sympathetic nerves on the wall of responsible blood vessels play an important role in the development of the disease. The purpose of this study was to investigate the role of norepinephrine (norepinephrine,NE) released by sympathetic nerve endings and its receptors in the pathogenesis of hemifacial spasm. Methods: the rat model of hemifacial spasm (hemifacial spasm,HFS) SD was established by Moller method. The stable abnormal muscle response wave (abnormal muscle response,AMR) was used as the successful standard. Then the facial nerve of experimental animals was stained with NE receptor immunohistochemical staining. The correlation between AMR wave and NE receptor expression was analyzed statistically. Then the facial nerve injury model was established by the same method. The NE receptor protein was detected by transmission electron microscope and Western-Blot. The correlation between the pathological changes of demyelinating of the epineurium and the expression of NE receptor protein was analyzed. Results: after the establishment of hemifacial spasm model, 6 out of 10 rats had AMR waves, and the immunohistochemical results indicated that the NE receptors in the facial nerve of all the 6 rats were ectopic and overexpression. However, only one of the other 4 rats with no AMR waveform showed ectopic expression of NE receptor (P0.05). The results of Western-Blot showed that the expression of NE receptor protein was increased in 6 of the 10 rats with nerve injury, and 7 of them were demyelinated in the epineurium of the facial nerve, and the results of Western-Blot showed that the expression of NE receptor was increased in 6 of the 7 rats. However, there was no significant change in the expression of NE receptor protein in the other 3 rats without injury to the epineurium of the facial nerve (P0.05 OR30.33). Conclusion: transmembrane NE receptor may appear after facial nerve outer membrane rupture, which plays an important role in the pathogenesis of hemifacial spasm.
