【摘要】：目的探討經(jīng)側裂入路顯微手術治療基底節(jié)區(qū)高血壓腦出血與常規(guī)顱骨開瓣手術治療的臨床療效差異。方法選取2013年12月至2016年6月我科治療80例基底節(jié)區(qū)高血壓腦出血患者,根據(jù)基底節(jié)區(qū)腦出血情況隨機篩選,經(jīng)側裂入路顯微手術治療為實驗組,常規(guī)顱骨開瓣手術治療為對照組,觀察比較兩組并發(fā)癥情況,患者日常生活能力(ADL)評估。結果實驗組并發(fā)癥率10%,對照組為35%,實驗組明顯低于對照組,差異有統(tǒng)計學意義(P0.05)。實驗組40例患者生存為38例,其中ADL 1~2級13例,3級15例,4級10例,死亡2例;對照組40例患者生存為36例,其中ADL 1~2級10例,3級11例,4級15例,死亡4例。實驗組手術恢復療效顯著優(yōu)于對照組,差異有統(tǒng)計學意義(P0.05)。結論經(jīng)側裂入路顯微手術有效清除治療基底節(jié)區(qū)高血壓腦,減輕神經(jīng)細胞損傷,降低并發(fā)癥發(fā)生率,值得臨床廣泛應用。
[Abstract]:Objective to investigate the clinical effects of microsurgery via lateral fissure approach in the treatment of hypertensive intracerebral hemorrhage in basal ganglia and routine craniotomy. Methods from December 2013 to June 2016, 80 patients with hypertensive intracerebral hemorrhage in basal ganglia were randomly selected and treated by microsurgery via lateral fissure approach. Routine craniotomy was performed in the control group. The complications of the two groups and the (ADL) evaluation of the patients' ADL were observed and compared. Results the complication rate was 10% in the experimental group and 35% in the control group, which was significantly lower in the experimental group than in the control group (P0.05). In the experimental group, 38 cases survived, including 13 cases of ADL grade 1, grade 2, 15 cases of grade 3, 15 cases of grade 4 and 2 cases of death, while the control group of 40 cases of survival was 36 cases, including ADL 1, grade 1, grade 2, grade 1, grade 3, grade 11, grade 4, death 4 cases. The effect of operation recovery in the experimental group was significantly better than that in the control group, the difference was statistically significant (P0.05). Conclusion the microsurgery via lateral fissure approach is effective in the treatment of hypertensive brain in basal ganglia, which can reduce nerve cell injury and reduce the incidence of complications, which is worthy of extensive clinical application.
