【摘要】：目的探討經(jīng)側(cè)裂入路顯微手術(shù)治療基底節(jié)區(qū)高血壓腦出血與常規(guī)顱骨開(kāi)瓣手術(shù)治療的臨床療效差異。方法選取2013年12月至2016年6月我科治療80例基底節(jié)區(qū)高血壓腦出血患者,根據(jù)基底節(jié)區(qū)腦出血情況隨機(jī)篩選,經(jīng)側(cè)裂入路顯微手術(shù)治療為實(shí)驗(yàn)組,常規(guī)顱骨開(kāi)瓣手術(shù)治療為對(duì)照組,觀察比較兩組并發(fā)癥情況,患者日常生活能力(ADL)評(píng)估。結(jié)果實(shí)驗(yàn)組并發(fā)癥率10%,對(duì)照組為35%,實(shí)驗(yàn)組明顯低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。實(shí)驗(yàn)組40例患者生存為38例,其中ADL 1~2級(jí)13例,3級(jí)15例,4級(jí)10例,死亡2例;對(duì)照組40例患者生存為36例,其中ADL 1~2級(jí)10例,3級(jí)11例,4級(jí)15例,死亡4例。實(shí)驗(yàn)組手術(shù)恢復(fù)療效顯著優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論經(jīng)側(cè)裂入路顯微手術(shù)有效清除治療基底節(jié)區(qū)高血壓腦,減輕神經(jīng)細(xì)胞損傷,降低并發(fā)癥發(fā)生率,值得臨床廣泛應(yīng)用。
[Abstract]:Objective to investigate the clinical effects of microsurgery via lateral fissure approach in the treatment of hypertensive intracerebral hemorrhage in basal ganglia and routine craniotomy. Methods from December 2013 to June 2016, 80 patients with hypertensive intracerebral hemorrhage in basal ganglia were randomly selected and treated by microsurgery via lateral fissure approach. Routine craniotomy was performed in the control group. The complications of the two groups and the (ADL) evaluation of the patients' ADL were observed and compared. Results the complication rate was 10% in the experimental group and 35% in the control group, which was significantly lower in the experimental group than in the control group (P0.05). In the experimental group, 38 cases survived, including 13 cases of ADL grade 1, grade 2, 15 cases of grade 3, 15 cases of grade 4 and 2 cases of death, while the control group of 40 cases of survival was 36 cases, including ADL 1, grade 1, grade 2, grade 1, grade 3, grade 11, grade 4, death 4 cases. The effect of operation recovery in the experimental group was significantly better than that in the control group, the difference was statistically significant (P0.05). Conclusion the microsurgery via lateral fissure approach is effective in the treatment of hypertensive brain in basal ganglia, which can reduce nerve cell injury and reduce the incidence of complications, which is worthy of extensive clinical application.
