【摘要】：目的觀察雙向等壓引流根治術(shù)治療高位單純性肛瘺的臨床療效。方法將120例高位單純性肛瘺并行手術(shù)治療的患者,按隨機(jī)數(shù)字表法分為治療組和對(duì)照組,每組60例。治療組行雙向等壓引流根治術(shù),對(duì)照組行高位掛線低位切開術(shù)。觀察術(shù)后24 h尿潴留評(píng)分、術(shù)后24 h及7天疼痛評(píng)分、創(chuàng)面愈合時(shí)間、治愈率,創(chuàng)面愈合后觀察Wexner失禁評(píng)分、SF-36生活質(zhì)量評(píng)分,隨訪6個(gè)月,觀察復(fù)發(fā)率。結(jié)果兩組患者的治愈率及術(shù)后6個(gè)月復(fù)發(fā)率比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。與對(duì)照組比較,治療組術(shù)后24 h尿潴留評(píng)分、術(shù)后24 h及7天疼痛程度評(píng)分均降低(P0.01),創(chuàng)面愈合時(shí)間縮短(P0.01),愈合后Wexner失禁評(píng)分降低(P0.01),SF-36生活質(zhì)量評(píng)分提高(P0.01)。結(jié)論與高位掛線低位切開術(shù)相比,雙向等壓引流根治術(shù)治療高位單純性肛瘺具有減輕局部疼痛、減少尿潴留發(fā)生、縮短療程、保護(hù)肛門功能、改善患者生活質(zhì)量優(yōu)勢。
[Abstract]:Objective to observe the clinical effect of double-way isobaric drainage in the treatment of high-level simple anal fistula. Methods 120 patients with high level simple anal fistula were divided into treatment group (n = 60) and control group (n = 60). Two-way isobaric drainage radical operation was performed in the treatment group and the high-hanging line-low incision was performed in the control group. Urinary retention score at 24 hours, pain score at 24 hours and 7 days after operation, wound healing time, cure rate, Wexner incontinence score and SF-36 quality of life score after wound healing were observed, followed up for 6 months, the recurrence rate was observed. Results there was no significant difference between the two groups in the cure rate and the recurrence rate at 6 months after operation (P0.05). Compared with the control group, the scores of urinary retention at 24 hours after operation, the pain degree at 24 hours and 7 days after operation in the treatment group were decreased (P0.01), the wound healing time was shortened (P0.01), the Wexner incontinence score was decreased (P0.01) and the quality of life score of SF-36 was improved (P0.01). Conclusion Bidirectional isobaric drainage radical operation in treating high level simple anal fistula has advantages of relieving local pain, reducing urinary retention, shortening course of treatment, protecting anal function and improving patients' quality of life.
