【摘要】：目的觀察月經(jīng)周期不同階段對(duì)婦科腹腔鏡手術(shù)后惡心嘔吐(PONV)的影響。方法選擇2016年3月至2017年2月在北京大學(xué)第一醫(yī)院接受全身麻醉下婦科腹腔鏡手術(shù)的患者228例。根據(jù)患者在手術(shù)日處于月經(jīng)周期的不同階段分為卵泡期(n=98)、排卵期(n=79)和黃體期(n=51)。收集圍術(shù)期資料,記錄術(shù)后0~2h、0~24h內(nèi)惡心嘔吐的發(fā)生情況。采用Logistic回歸模型分析月經(jīng)周期不同階段對(duì)PONV發(fā)生率的影響。結(jié)果術(shù)后0~2h有53例(23.2%)患者發(fā)生PONV,0~24h有125例(54.8%)PONV。單因素分析顯示:月經(jīng)周期不同階段PONV發(fā)生率差異無(wú)統(tǒng)計(jì)學(xué)意義。多因素Logistic回歸分析顯示:手術(shù)時(shí)間1h(OR=3.176,95%CI 1.567~6.436,P=0.001)、PONV史(OR=5.711,95%CI 1.710~19.080,P=0.005)是術(shù)后0~2h惡心嘔吐的獨(dú)立危險(xiǎn)因素;手術(shù)時(shí)間1h(OR=2.577,95%CI 1.460~4.549,P=0.001)、術(shù)后使用PCA泵(OR=2.671,95%CI 1.224~5.831,P=0.014)、術(shù)后應(yīng)用甲硝唑(OR=2.728,95%CI 1.413~5.267,P=0.003)是術(shù)后0~24h惡心嘔吐的獨(dú)立危險(xiǎn)因素。結(jié)論月經(jīng)周期不同階段對(duì)婦科腹腔鏡手術(shù)后惡心嘔吐的發(fā)生率無(wú)明顯影響。
[Abstract]:Objective to observe the effect of different periods of menstrual cycle on (PONV) of nausea and vomiting after gynecological laparoscopy. Methods 228 patients undergoing gynecologic laparoscopic surgery under general anesthesia were selected from March 2016 to February 2017 in the first Hospital of Peking University. According to the different stages of menstrual cycle on the day of operation, the patients were divided into follicular stage (nm98), ovulation stage (nm79) and luteal phase (N51). The data of perioperative period were collected to record the incidence of nausea and vomiting within 2 hours and 24 hours after operation. Logistic regression model was used to analyze the influence of different periods of menstrual cycle on the incidence of PONV. Results there were 53 cases (23.2%) with PONV,0~24h at 0 h after operation, 125 cases (54.8%) with PONV..
