【摘要】：目的評(píng)估術(shù)前靜脈滴注氨甲環(huán)酸對(duì)老年股骨粗隆間骨折患者行股骨近端髓內(nèi)釘-螺旋刀片(PFNA)手術(shù)圍術(shù)期失血的影響。方法選擇股骨粗隆間骨折行PFNA內(nèi)固定的老年患者115例,分為治療組(58例)和對(duì)照組(57例)。治療組術(shù)前30min靜脈滴注20mg/kg氨甲環(huán)酸,對(duì)照組無(wú)特殊處理。記錄兩組術(shù)前,術(shù)后第1、3天血紅蛋白(Hb)及紅細(xì)胞壓積(Hct)情況,通過(guò)Gross方程與Nadler方程計(jì)算圍術(shù)期總失血量,統(tǒng)計(jì)兩組患者輸血率、血栓性并發(fā)癥發(fā)生情況。結(jié)果治療組圍術(shù)期總失血量(872.21±312.53)mL,對(duì)照組(1 162.41±368.64)mL,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。治療組輸血率3.45%,對(duì)照組17.5%,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。兩組患者均無(wú)深靜脈血栓及肺栓塞發(fā)生。治療組肌間靜脈血栓發(fā)生率5.1%,對(duì)照組7.0%,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論老年股骨粗隆間骨折患者行PFNA內(nèi)固定術(shù)前靜脈滴注20mg/kg氨甲環(huán)酸能有效減少圍術(shù)期失血,并且不增加血栓形成風(fēng)險(xiǎn)。
[Abstract]:Objective to evaluate the effect of intravenously instilled methoxycycline on perioperative blood loss in elderly patients with intertrochanteric fracture of femur treated with proximal femoral intramedullary nail and spiral blade (PFNA). Methods 115 elderly patients with intertrochanteric fracture treated with PFNA were divided into treatment group (58 cases) and control group (57 cases). 30min was intravenously infused with 20mg/kg in the treatment group and no special treatment was found in the control group. Hemoglobin (Hb) and hematocrit (Hct) were recorded 3 days before and 1 day after operation in both groups. The total blood loss was calculated by Gross equation and Nadler equation. The blood transfusion rate and thrombotic complications were calculated in both groups. Results the total blood loss in the treatment group (872.21 鹵312.53) mL, control group (1 162.41 鹵368.64) mL, was significantly different (P0.01). The blood transfusion rate was 3.45 in the treatment group and 17.5 in the control group, the difference was statistically significant (P 0.01). No deep venous thrombosis or pulmonary embolism occurred in both groups. The incidence of intramuscular venous thrombosis in the treatment group was 5.1 and that in the control group was 7.0. The difference was not statistically significant (P0.05). Conclusion intravenous infusion of 20mg/kg in elderly patients with intertrochanteric fracture of femur before PFNA fixation can effectively reduce perioperative blood loss and does not increase the risk of thrombosis.
