【摘要】：目的觀察術(shù)中靜脈使用氨甲環(huán)酸對(duì)于初次、單側(cè)膝關(guān)節(jié)表面置換術(shù)并且術(shù)后采用自體血回輸?shù)脑擃惒±闹寡Ч鞍踩栽u(píng)價(jià)。方法采用隨機(jī)對(duì)照的前瞻性研究方法,自2015年1月至2016年2月,共有124例納入研究,將其分為試驗(yàn)組(氨甲環(huán)酸靜脈滴注)和對(duì)照組(生理鹽水滴注),分別記錄及比較兩組的術(shù)后引流量、自體血回輸量、術(shù)后血紅蛋白及紅細(xì)胞壓積、術(shù)后凝血指標(biāo)、總失血量及隱性失血量、異體輸血例數(shù)以及輸血量。結(jié)果試驗(yàn)組術(shù)后6h引流量及自體血回輸量較對(duì)照組少,術(shù)后1、3d血紅蛋白及紅細(xì)胞壓積降低值小,術(shù)后1dD二聚體較低,術(shù)后總失血量及隱性失血量較小,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。試驗(yàn)組有1例需要輸異體血,明顯低于對(duì)照組(6例)。兩組均無(wú)深靜脈血栓及其他重大并發(fā)癥出現(xiàn)。結(jié)論在初次、單側(cè)膝關(guān)節(jié)置換術(shù)中使用止血帶及不特殊止血,術(shù)后采用自體血回輸?shù)幕颊咧?關(guān)閉切口時(shí)靜脈使用氨甲環(huán)酸(15mg/kg)能夠有效減少總失血量,特別是術(shù)后早期引流量,能夠降低異體血輸血比例并且不增加深靜脈血栓的風(fēng)險(xiǎn)。
[Abstract]:Objective to evaluate the hemostatic effect and safety of intraoperative intravenous use of carbamoic acid in patients undergoing primary, unilateral knee joint surface replacement and postoperative autologous blood transfusion. Methods from January 2015 to February 2016, 124 patients were enrolled in a prospective randomized controlled study. The patients were divided into two groups: the experimental group and the control group. The postoperative drainage volume, autologous blood volume, hemoglobin and hematocrit were recorded and compared between the two groups. Total and recessive blood loss, cases of allogeneic blood transfusion and blood transfusion volume. Results the drainage volume and autologous blood volume in the test group were less than those in the control group, the hemoglobin and hematocrit decreased at 1 ~ 3 days after operation, the 1dD dimer was lower, the total blood loss and the recessive blood loss were smaller in the experimental group than in the control group (P0.05). One case in the test group needed allogeneic blood transfusion, which was significantly lower than that in the control group (6 cases). There were no deep venous thrombosis and other major complications in both groups. Conclusion in the primary, unilateral knee arthroplasty patients who used tourniquet or no special hemostasis and who received autologous blood transfusion after operation, intravenous use of 15mg/kg at the time of closure of the incision could effectively reduce the total bleeding loss, especially the early postoperative drainage. It can reduce the proportion of allogeneic blood transfusion without increasing the risk of deep venous thrombosis.
