羥考酮聯(lián)合右美托咪定在纖維支氣管鏡清醒氣管插管中的應(yīng)用

【摘要】：目的觀察羥考酮復(fù)合右美托咪定在清醒氣管插管中的鎮(zhèn)痛鎮(zhèn)靜效果以及對呼吸和循環(huán)功能的影響。方法選擇術(shù)前預(yù)計(jì)困難氣道的擇期手術(shù)患者40例,隨機(jī)分為舒芬太尼組和羥考酮組。兩組患者靜脈泵注右美托咪定1μg/kg后以0.2μg·kg~(-1)·h~(-1)維持,舒芬太尼組靜注舒芬太尼0.2μg/kg,羥考酮組靜注羥考酮0.08mg/kg。監(jiān)測患者用藥及插管期間HR、MAP和SpO_2變化,并記錄插管前Ramsay評分以及插管后PETCO_2和舒適度評分。結(jié)果兩組患者用藥及插管期間的血流動(dòng)力學(xué)變化、鎮(zhèn)靜及舒適度差異無統(tǒng)計(jì)學(xué)意義。羥考酮組患者靜注后SpO_2明顯高于舒芬太尼組(P0.05),氣管插管后PETCO_2明顯降低(P0.05)。結(jié)論羥考酮復(fù)合右美托咪定可安全地用于清醒氣管插管,鎮(zhèn)痛、鎮(zhèn)靜效果良好,與舒芬太尼相比,呼吸抑制輕,不良反應(yīng)少。
[Abstract]:Objective to observe the analgesic and sedative effects of hydroxycodone combined with dexmetidine on respiratory and circulatory function in conscious tracheal intubation. Methods 40 patients with difficult airway were randomly divided into sufentanil group and hydroxycodone group. After intravenous injection of dexmetidine (1 渭 g/kg), the patients in both groups were maintained with 0.2 渭 g kg~ (-1) h-1, sufentanil 0.2 渭 g / kg, hydroxycodone 0.08 mg / kg. The changes of HR,MAP and SpO_2 during medication and intubation were monitored and Ramsay scores before intubation and PETCO_2 and comfort scores after intubation were recorded. Results there was no significant difference in hemodynamics, sedation and comfort between the two groups during medication and intubation. The SpO_2 of hydroxycodone group was significantly higher than that of sufentanil group (P0.05), and the PETCO_2 was significantly decreased after endotracheal intubation (P0.05). Conclusion hydroxycodone combined with dexmetidine can be safely used in conscious tracheal intubation, analgesia and sedation. Compared with sufentanil, the respiratory inhibition is less and the adverse reactions are less.
