應用高屈曲旋轉平臺型假體行全膝關節(jié)置換的近期療效

【摘要】:背景人工全膝關節(jié)置換術在臨床治療上已經成熟。高屈曲旋轉平臺型膝關節(jié)假體在設計上更具優(yōu)勢,更加接近膝關節(jié)生理結構。目的探討應用LPS-Flex Mobile Bearing System人工膝假體(Zimmer,USA)行全膝關節(jié)置換的近期療效。方法回顧性分析2012-02/2014-03蘇州大學附屬第一醫(yī)院收治的行Zimmer LPS-Flex Mobile Bearing System人工膝假體置換的患者37例(42膝),男9例(10膝),女28例(32膝),年齡47-78歲,平均63.7歲;置換術前診斷:骨性關節(jié)炎35例(39膝),類風濕性關節(jié)炎2例(3膝)。采用骨水泥固定,不進行髕骨置換;觀察術后并發(fā)癥,比較術前、術后膝關節(jié)活動度,并根據HSS膝關節(jié)評分系統(tǒng)進行評估。結果獲隨訪34例(38膝),時間6-28(平均15.14)個月。活動度從術前平均88.5改善到術后平均124.2;術前HSS評分平均為52.5分,術后HSS評分平均為91.1分;均較術前有明顯改善(P0.01)。根據HSS評分系統(tǒng)評定療效:優(yōu)20例,良16例,中2例,優(yōu)良率達94.74%。各種并發(fā)癥的發(fā)生率低。到最后一次隨訪為止,所有病例均未出現(xiàn)下肢深靜脈血栓形成、假體周圍骨折、髕骨撞擊等并發(fā)癥。置換后隨訪X射線片均未顯示任何骨溶解、關節(jié)脫位、假體-骨界面的X射線透亮帶、以及與松動相關的假體力線改變等X射線征。結論高屈曲旋轉平臺型人工全膝關節(jié)置換的近期臨床效果滿意,長期效果還需進一步觀察。
[Abstract]:Background Total knee arthroplasty is mature in clinical treatment. The high-flexion rotary platform knee prosthesis has more advantages in design and is closer to the physiological structure of the knee joint. Objective to investigate the short-term effect of total knee arthroplasty with LPS-Flex Mobile Bearing System artificial knee prosthesis (Zimmer,USA). Methods A retrospective analysis was made on 37 patients (42 knees) treated with Zimmer LPS-Flex Mobile Bearing System prosthesis replacement in the first affiliated Hospital of Suzhou University, 2012-02 / 2014-03, including 9 males (10 knees) and 28 females (32 knees), aged 47-78 years (mean 63.7 years). 35 cases (39 knees) of osteoarthritis and 2 cases (3 knees) of rheumatoid arthritis were diagnosed before replacement. Using bone cement fixation without patellar replacement, the postoperative complications were observed, the preoperative and postoperative knee motion were compared, and evaluated according to the HSS knee scoring system. Results 34 cases (38 knees) were followed up for 6-28 months (mean 15.14 months). The average range of activity was improved from 88.5 before operation to 124.2 after operation, the mean HSS score was 52.5 before operation, and the average HSS score after operation was 91.1, which was significantly improved compared with that before operation (P0.01). According to the HSS scoring system, 20 cases were excellent, 16 cases were good, 2 cases were fair, and the excellent and good rate was 94.74. The incidence of various complications was low. Up to the last follow-up, there were no complications such as deep vein thrombosis, periprosthetic fracture and patellar impact in all cases. There were no X-ray signs such as osteolysis, dislocation of joint, X-ray clear zone at the prosthetic bone interface, and pseudo physical force changes associated with loosening. Conclusion the short-term clinical effect of high-flexion rotation platform total knee arthroplasty is satisfactory, and the long-term effect needs further observation.
