MRS定量評價腰椎間盤退變

【摘要】:目的探討MRS技術(shù)定量評價腰椎間盤退變程度的價值。方法對82例腰痛患者行腰椎MR常規(guī)掃描,采用T2WI矢狀位圖像進(jìn)行Pfirrmann分級;采用MRS技術(shù)檢測腰椎間盤髓核的氮-乙酰氨基(N-acetyl)峰下面積、水(Water)峰下面積及二者峰下面積比值,分析MRS測量值與Pfirrmann退變分級、患者年齡間的相關(guān)性。結(jié)果 82例患者中,204個椎間盤可獲得MRS測量值,其中Ⅱ、Ⅲ、Ⅳ、Ⅴ級椎間盤數(shù)目分別為89個、73個、39個、3個,髓核區(qū)N-acetyl峰下面積、Water峰下面積、N-acetyl/Water峰下面積比值與腰椎間盤Pfirrmann分級呈負(fù)相關(guān)(r_s=-0.460、-0.204、-0.526,P均0.05)。30歲、30~39歲、40~49歲、50~59歲、59歲的患者中,取得MRS測量值的椎間盤數(shù)目分別為62個、25個、37個、51個、29個,腰椎間盤髓核區(qū)N-acetyl峰下面積、N-acetyl/Water峰下面積比值與年齡段呈負(fù)相關(guān)(r_s=-0.247、-0.385,P均0.05)。結(jié)論 MRS技術(shù)可用以定量檢測腰椎間盤的退變程度。
