【摘要】：目的探討MRS技術(shù)定量評(píng)價(jià)腰椎間盤(pán)退變程度的價(jià)值。方法對(duì)82例腰痛患者行腰椎MR常規(guī)掃描,采用T2WI矢狀位圖像進(jìn)行Pfirrmann分級(jí);采用MRS技術(shù)檢測(cè)腰椎間盤(pán)髓核的氮-乙酰氨基(N-acetyl)峰下面積、水(Water)峰下面積及二者峰下面積比值,分析MRS測(cè)量值與Pfirrmann退變分級(jí)、患者年齡間的相關(guān)性。結(jié)果 82例患者中,204個(gè)椎間盤(pán)可獲得MRS測(cè)量值,其中Ⅱ、Ⅲ、Ⅳ、Ⅴ級(jí)椎間盤(pán)數(shù)目分別為89個(gè)、73個(gè)、39個(gè)、3個(gè),髓核區(qū)N-acetyl峰下面積、Water峰下面積、N-acetyl/Water峰下面積比值與腰椎間盤(pán)Pfirrmann分級(jí)呈負(fù)相關(guān)(r_s=-0.460、-0.204、-0.526,P均0.05)。30歲、30~39歲、40~49歲、50~59歲、59歲的患者中,取得MRS測(cè)量值的椎間盤(pán)數(shù)目分別為62個(gè)、25個(gè)、37個(gè)、51個(gè)、29個(gè),腰椎間盤(pán)髓核區(qū)N-acetyl峰下面積、N-acetyl/Water峰下面積比值與年齡段呈負(fù)相關(guān)(r_s=-0.247、-0.385,P均0.05)。結(jié)論 MRS技術(shù)可用以定量檢測(cè)腰椎間盤(pán)的退變程度。
