【摘要】：目的本研究系統(tǒng)評價及綜述肝移植患者術(shù)后呼吸機相關(guān)性肺炎的發(fā)病率及危險因素,為預(yù)防決策提供依據(jù)。方法計算機檢索Embase,Pubmed,Web of science,cochrane library,維普,萬方,知網(wǎng),中國生物醫(yī)學(xué)文獻數(shù)據(jù)庫數(shù)據(jù)庫(Sinomed),檢索時間截止為2016年11月。結(jié)果Meta分析結(jié)果顯示,肝移植術(shù)后呼吸機相關(guān)性肺炎的發(fā)病率為10%,死亡率為38.5%。肝移植術(shù)后呼吸機相關(guān)性肺炎的重要感染因素按照OR值大小及(95%CI)依次為免疫制劑6.58[2.28,9.89],ICU時間2天5.21[3.46,8.95],呼吸機使用時間2天4.54[1.77,6.31],術(shù)后MELD評分15分3.98[1.06,6.21]等。結(jié)論肝移植術(shù)后呼吸機相關(guān)性肺炎發(fā)病率較高,應(yīng)針對各項危險因素,采取相應(yīng)的預(yù)防措施,從而降低感染的發(fā)生。
[Abstract]:Objective to evaluate and summarize the incidence and risk factors of ventilator-associated pneumonia in patients with liver transplantation. Methods the retrieval time of Embase Pubmedan web of science cochrane library, Weip, Wanfang, Zhiwang, and Chinese biomedical literature database (Sinomed),) was searched by computer in November, 2016. Results Meta analysis showed that the incidence of ventilator-associated pneumonia after liver transplantation was 10 and the mortality was 38.5. According to OR value and (95%CI), the important infection factors of ventilator-associated pneumonia after liver transplantation were immune agent 6.58 [2.289.89] ICU time 5.21 [3.468.95], ventilator service time 2.54 [1.776.31], postoperative MELD score 15 ~ 3.98 [1.066.21] and so on. Conclusion the incidence of ventilator-associated pneumonia after liver transplantation is high.
