【摘要】：目的探討骨髓干細(xì)胞移植聯(lián)合傘狀支撐骨移植術(shù)治療股骨頭壞死(osteonecrosis of the femoral head,ONFH)的臨床效果。方法 ONFH患者48例(58髖),其中30例(36髖)采用骨髓干細(xì)胞和傘狀支撐骨移植術(shù)為觀察組,18例(22髖)行單純股方肌骨柱移植術(shù)為對照組;比較2組術(shù)后百分法評分和影像學(xué)檢查結(jié)果。結(jié)果觀察組與對照組術(shù)前百分法評分[(56.06±6.88)、(55.54±6.36)分]比較差異無統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后隨訪18個(gè)月,2組百分法評分均較術(shù)前提高,且觀察組評分[(87.47±5.66)分]高于對照組[(76.14±7.57)分],治療優(yōu)良率(91.67%)高于對照組(72.7%)(P0.05);觀察組術(shù)后植骨愈合時(shí)間[(3.9±0.5)個(gè)月]和MRI壞死面積百分比[(9.19±5.84)%]低于對照組[(5.2±0.6)個(gè)月、(14.05±5.96)%],差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論骨髓干細(xì)胞和傘狀支撐骨移植術(shù)治療ONFH可加速成骨,更好恢復(fù)股骨頭的生物力學(xué)、防止股骨頭塌陷,近期效果良好。
[Abstract]:Objective to investigate the clinical effect of bone marrow stem cell transplantation combined with umbrella support bone graft in the treatment of femoral head osteonecrosis (osteonecrosis of the femoral head ONFH). Methods 48 patients (58 hips) with ONFH were treated with bone marrow stem cells (BMSCs) and umbrella-supported bone graft (UBSO) in 30 cases (36 hips) as the control group (n = 18). The score and imaging results of the two groups were compared. Results there was no significant difference in the preoperative score between the observation group and the control group [(56.06 鹵6.88), (55.54 鹵6.36)] (P0.05). The score of the observation group [(87.47 鹵5.66)] was higher than that of the control group [(76.14 鹵7.57)], the excellent and good rate of treatment (91.67%) was higher than that of the control group (72.7%) (P0.05), the time of bone graft healing [(3.9 鹵0.5) months] and the percentage of MRI necrosis area [(9.19 鹵5.84)%] in the observation group were significantly lower than those in the control group [(5.2 鹵0.6) months, (14.05 鹵5.96)%] (P0.05). Conclusion Bone marrow stem cells and umbrella bone graft can accelerate osteogenesis, recover biomechanics of femoral head and prevent femoral head from collapsing.
