【摘要】：目的對(duì)比經(jīng)眶上外側(cè)入路和經(jīng)眶上鎖孔入路夾閉前循環(huán)動(dòng)脈瘤的治療效果,以指導(dǎo)前循環(huán)動(dòng)脈瘤手術(shù)入路的選擇。方法回顧性分析2011年1月至2016年1月因自發(fā)性蛛網(wǎng)膜下腔出血確診為前循環(huán)動(dòng)脈瘤80例患者的臨床資料,分為經(jīng)眶上鎖孔入路(A組)和眶上外側(cè)入路(B組)。分析兩組患者的手術(shù)時(shí)間、開(kāi)顱時(shí)間、關(guān)顱時(shí)間、估計(jì)術(shù)中出血量、手術(shù)前后紅細(xì)胞壓積(HCT)改變、術(shù)后早期下床活動(dòng)時(shí)間、術(shù)后住院時(shí)間、入院和出院時(shí)格拉斯哥昏迷評(píng)分(GCS評(píng)分)、出院時(shí)格拉斯哥預(yù)后評(píng)分(GOS評(píng)分)的差異并進(jìn)行文獻(xiàn)回顧。結(jié)果經(jīng)眶上外側(cè)入路的手術(shù)時(shí)間和開(kāi)顱時(shí)間少于經(jīng)眶上鎖孔入路,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組關(guān)顱時(shí)間、估計(jì)術(shù)中出血量、手術(shù)前后HCT改變、術(shù)后早期下床活動(dòng)時(shí)間、術(shù)后住院時(shí)間、入院和出院時(shí)GCS評(píng)分、出院時(shí)GOS評(píng)分差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論經(jīng)眶上外側(cè)入路具有手術(shù)時(shí)間和關(guān)顱時(shí)間短,術(shù)中能提供更好的手術(shù)視野,術(shù)中牽拉較少和術(shù)后并發(fā)癥少等優(yōu)點(diǎn),在行顱內(nèi)前循環(huán)動(dòng)脈瘤夾閉術(shù)時(shí)可作為眶上鎖孔入路的替代方案。
[Abstract]:Objective to compare the therapeutic effect of anterior circulation aneurysm clipping via supraorbital lateral approach and supraorbital keyhole approach in order to guide the choice of surgical approach for anterior circulation aneurysm. Methods the clinical data of 80 patients with anterior circulatory aneurysm diagnosed as spontaneous subarachnoid hemorrhage from January 2011 to January 2016 were retrospectively analyzed. The patients were divided into subeorbital keyhole approach (group A) and supraorbital lateral approach (group B). The time of operation, time of craniotomy, time of closing cranium, estimation of intraoperative bleeding, changes of hematocrit (HCT) before and after operation, time of getting out of bed early after operation, time of hospitalization after operation were analyzed. The differences of Glasgow coma score (GCS) at admission and discharge and Glasgow prognosis score (GOS) at discharge were reviewed. Results the operative time and craniotomy time of transorbital lateral approach was less than that of transorbital keyhole approach (P0.05). There was no significant difference between the two groups in the time of cranial closure, the estimated amount of intraoperative bleeding, the changes of HCT before and after operation, the time of getting out of bed early after operation, the time of hospitalization, the GCS score at admission and discharge, and the GOS score at discharge (P0.05). Conclusion Transorbital approach has the advantages of short operative time and closed cranial time, better operative visual field, less traction and less postoperative complications. Intracranial anterior circulation aneurysm clipping can be used as an alternative to supraorbital keyhole approach.
