【摘要】：目的觀察基于骨折塊大小的空心拉力螺釘固定治療Bennett骨折的效果。方法以2014年3月~2015年10月在我院接受治療的Bennett骨折患者為觀察對(duì)象。根據(jù)其治療方式分為經(jīng)皮克氏針內(nèi)固定組和空心拉力螺釘固定組,觀察兩組患者骨折愈合時(shí)間,手術(shù)前后VAS得分和腕關(guān)節(jié)功能的差異。結(jié)果兩組患者骨折愈合時(shí)間無明顯差別,而空心拉力螺釘固定組患者術(shù)后VAS得分明顯低于克氏針內(nèi)固定組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者治療前腕關(guān)節(jié)屈伸活動(dòng)、尺橈偏活動(dòng)、旋前旋后活動(dòng)、平均握力和平均捏力與健側(cè)百分比無明顯差別,治療后,兩組患者的上述指標(biāo)均較治療前增高,且空心拉力螺釘固定組增高更明顯(P0.05);兩組患者治療前腕關(guān)節(jié)功能評(píng)分無明顯差別,治療后,兩組患者的腕關(guān)節(jié)功能評(píng)分均增高,且空心拉力螺釘固定組增高更明顯(P0.05)。結(jié)論基于骨折塊大小的空心拉力螺釘固定對(duì)Bennett骨折有較好的治療效果,可明顯改善患者的腕關(guān)節(jié)功能。
[Abstract]:Objective to observe the effect of hollow lag screw fixation on Bennett fracture. Methods Bennett fracture patients who were treated in our hospital from March 2014 to October 2015 were studied. According to the treatment method, the patients were divided into two groups: percutaneous Kirschner needle fixation group and hollow lag screw group. The fracture healing time, VAS score and wrist function were observed before and after operation. Results there was no significant difference in fracture healing time between the two groups, but the VAS score in the hollow lag screw fixation group was significantly lower than that in the Kirschner needle fixation group (P0.05). There was no significant difference between the radial deviation of ulnar movement, pronation and pronation, the average grip strength and the percentage of the contralateral side. After treatment, the above indexes in the two groups were higher than those before treatment. The carpal function score of the two groups before treatment was not significant difference, after treatment, the wrist function score of the two groups were increased, and the increase of the hollow lag screw fixation group was more obvious (P0.05). Conclusion Hollow pull screw fixation based on the size of fracture block has a good therapeutic effect on Bennett fracture and can obviously improve the wrist joint function of the patients.
