【摘要】：目的:通過自體脂肪微片段關(guān)節(jié)注射與玻璃酸鈉關(guān)節(jié)注射兩種不同方法治療膝骨關(guān)節(jié)炎后的療效進(jìn)行對(duì)照觀察,了解自體脂肪微片段治療膝骨關(guān)節(jié)炎的療效。方法:將滿足納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)的57名受試者隨機(jī)分成兩組。分別使用自體脂肪微片段和玻璃酸鈉注射治療膝骨關(guān)節(jié)炎。分別記錄兩組治療前、治療后1周、1個(gè)月、3個(gè)月及6個(gè)月的觀察指標(biāo),進(jìn)行數(shù)據(jù)整理,使用統(tǒng)計(jì)學(xué)方法進(jìn)行數(shù)據(jù)分析后得出結(jié)論。結(jié)果:兩組內(nèi)在隨訪至治療后3個(gè)月時(shí)治療不同時(shí)間KOOS評(píng)分與前一次相比均有顯著的差異(P0.05);但在治療后6個(gè)月時(shí),玻璃酸鈉組較治療后3個(gè)月無顯著差異,而自體脂肪微片段仍有顯著差異。玻璃酸鈉組內(nèi)在治療后1周、治療后1月VAS評(píng)分較前一次有顯著差異(P0.05),且均值較前一次VAS評(píng)分小;在治療后3月、治療后6月分別與前一次VAS評(píng)分相比也有顯著差異(P0.05),但均值較前一次VAS評(píng)分大。而在自體脂肪微片段組內(nèi)只在隨訪6個(gè)月時(shí)與治療后3個(gè)月相比較無顯著差異。在兩種治療方法的比較中,治療1周后,兩組KOOS評(píng)分無顯著差異(P0.05);VAS評(píng)分存在顯著差異(P0.05)。在治療1月后兩組KOOS評(píng)分有顯著差異(P0.05);VAS評(píng)分無顯著差異(P0.05)。治療3月及6月后兩組KOOS評(píng)分均有顯著差異(P0.05);VAS評(píng)分均有顯著差異(P0.05)。治療6個(gè)月后自體脂肪微片段組HSS優(yōu)良率81.5%大于玻璃酸鈉組HSS優(yōu)良率73.3%。結(jié)論:與關(guān)節(jié)腔內(nèi)玻璃酸鈉注射相比行關(guān)節(jié)腔內(nèi)自體脂肪微片段注射對(duì)緩解膝骨關(guān)節(jié)疼痛雖然起效較慢,但持續(xù)時(shí)間長且明顯,而在改善膝關(guān)節(jié)功能方面效果也更顯著。
[Abstract]:Objective: to observe the curative effect of autogenous fat microfragment injection and sodium hyaluronate injection on knee osteoarthritis and to understand the effect of autologous fat microfragment in the treatment of knee osteoarthritis. Methods: 57 subjects who met the inclusion criteria and exclusion criteria were randomly divided into two groups. Autogenous fat microfragments and sodium hyaluronate injection were used to treat knee osteoarthritis respectively. The observation indexes before treatment, 1 week, 1 month, 3 months and 6 months after treatment were recorded, and the data were sorted out. Results: there was significant difference in KOOS score between the two groups at different time after treatment from 3 months to 3 months after treatment (P0.05), but there was no significant difference in sodium hyaluronate group at 6 months after treatment as compared with 3 months after treatment. However, there were still significant differences in autologous fat microfragments. The VAS score of sodium hyaluronate group was significantly different from that of the previous group at 1 week after treatment (P0.05), and the mean value was lower than that of the previous VAS score. Six months after treatment, there were significant differences between the previous VAS score and the previous VAS score (P0.05), but the mean value was larger than the previous VAS score. However, there was no significant difference between autologous fat microfragment group and 3 months after treatment at 6 months follow-up. After one week of treatment, there was no significant difference in KOOS score between the two groups (P0.05). After one month of treatment, there was significant difference in KOOS score between the two groups (P0.05). There was no significant difference in VAS score between the two groups (P0.05). After 3 months and 6 months of treatment, there were significant differences in KOOS scores between the two groups (P0.05). After 6 months of treatment, the excellent and good rate of HSS in autologous fat microfragment group was 81.5%, that in sodium hyaluronate group was 73.3%, and that in sodium hyaluronate group was 73.3%. Conclusion: compared with intraarticular injection of sodium hyaluronate, intra-articular autogenous fat microinjection can alleviate the pain of knee joint more slowly, but it lasts for a long time, and it is more effective in improving the function of knee joint.
