【摘要】：目的探討成人心肺轉(zhuǎn)流(cardiopulmonary bypass,CPB)下心臟瓣膜手術(shù)后急性腎損傷(acute kidney injury,AKI)的危險(xiǎn)因素。方法回顧性分析1 349例心臟瓣膜手術(shù)患者的臨床資料,采用多因素Logistic回歸分析心臟瓣膜術(shù)后AKI的危險(xiǎn)因素。結(jié)果 1 349例心臟瓣膜手術(shù)患者AKI發(fā)生率為28.4%,多因素Logistic回歸分析顯示,每增加1歲(OR=1.05,95%CI 1.03~1.06,P0.001)、糖尿病史(OR=2.11,95%CI 1.22~3.68,P=0.008)、貧血(OR=1.50,95%CI1.05~2.21,P=0.026)、術(shù)前血清肌酐(Scr)值每增加1mg/dl(OR=1.01,95%CI 1.01~1.02,P=0.001)、手術(shù)時(shí)間每增加1h(OR=1.28,95%CI 1.15~1.41,P0.001)、術(shù)中輸注血漿(OR=1.50,95%CI 1.14~1.97,P=0.004)是心臟瓣膜術(shù)后發(fā)生AKI的獨(dú)立危險(xiǎn)因素。結(jié)論心肺轉(zhuǎn)流下心臟瓣膜術(shù)后急性腎損傷的獨(dú)立危險(xiǎn)因素是高齡、糖尿病史、貧血、術(shù)前肌酐高、手術(shù)時(shí)間長(zhǎng)以及術(shù)中輸注血漿。
[Abstract]:Objective to investigate the risk factors of acute renal injury (acute kidney injury-AKI) after cardiac valve surgery in adult patients with cardiopulmonary bypass (cardiopulmonary). Methods the clinical data of 1 349 patients undergoing cardiac valve surgery were retrospectively analyzed. Multivariate Logistic regression analysis was used to analyze the risk factors of AKI after cardiac valve surgery.
