【摘要】：目的:探討復(fù)方麝香注射液聯(lián)合依達(dá)拉奉對(duì)顱腦外傷患者凝血功能、神經(jīng)功能的影響及安全性。方法:選取我院2015年4月-2016年4月收治的顱腦外傷患者84例,按照隨機(jī)數(shù)字表法分為觀(guān)察組與對(duì)照組,各42例。對(duì)照組患者給予依達(dá)拉奉注射液30 mg加入0.9%氯化鈉注射液100 m L中,bid,ivgtt;觀(guān)察組在對(duì)照組基礎(chǔ)上加用復(fù)方麝香注射液20 m L加入0.9%氯化鈉注射液250 m L中,qd,ivgtt。兩組患者均治療4周。比較兩組患者治療前后神經(jīng)功能缺損程度(NIHSS)評(píng)分、格拉斯昏迷分級(jí)(GCS)評(píng)分、血清神經(jīng)元特異性烯醇化酶(NSE)水平、C反應(yīng)蛋白(CRP)水平、凝血酶原時(shí)間(PT)、活化部分凝血活酶時(shí)間(APTT)、D-二聚體水平、格拉斯預(yù)后分級(jí)(GOS)評(píng)分及不良反應(yīng)發(fā)生情況。結(jié)果:治療后,兩組患者NIHSS評(píng)分均較治療前顯著降低,且觀(guān)察組顯著低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者GCS評(píng)分均較治療前顯著增高,且觀(guān)察組顯著高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者血清NSE、CRP、PT、APTT和D-二聚體水平均較治療前顯著降低,且觀(guān)察組顯著低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);觀(guān)察組患者GOS評(píng)分顯著高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者治療期間未見(jiàn)明顯不良反應(yīng)發(fā)生。結(jié)論:復(fù)方麝香注射液聯(lián)合依達(dá)拉奉可改善顱腦外傷患者凝血功能和神經(jīng)功能,且可減輕患者顱腦損傷嚴(yán)重程度,改善預(yù)后。
[Abstract]:Objective: to investigate the effect and safety of compound musk injection combined with Edaravone on blood coagulation and nerve function in patients with craniocerebral trauma. Methods: 84 patients with craniocerebral trauma admitted in our hospital from April 2015 to April 2016 were randomly divided into two groups: observation group (n = 42) and control group (n = 42). The patients in the control group were given 30 mg Edaravone injection and 0.9% sodium chloride injection 100 mL respectively, and those in the observation group were treated with compound musk injection 20 mL and 0.9% sodium chloride injection 250 mL in the observation group. Both groups were treated for 4 weeks. Before and after treatment, (NIHSS) score, (GCS) score, (NSE) level of neuron-specific enolase and C-reactive protein (CRP) were compared between the two groups. Prothrombin time (PT),) activated partial thromboplastin time (APTT) level of D-dimer, (GOS) score of Glass-Prognostic grading and occurrence of adverse reactions. Results: after treatment, the NIHSS scores of the two groups were significantly lower than those of the control group (P0.05), the GCS scores of the two groups were significantly higher than those of the control group, and the GCS scores of the observation group were significantly higher than those of the control group. The serum levels of NSE-CRPP-PTAPTT and D- dimer in the two groups were significantly lower than those in the control group (P0.05), and the GOS score of the observation group was significantly higher than that of the control group (P0.05). The difference was statistically significant (P0.05). No significant adverse reactions occurred during treatment in both groups. Conclusion: compound musk injection combined with Edaravone can improve blood coagulation function and nerve function in patients with craniocerebral trauma, reduce the severity of craniocerebral injury and improve prognosis.
