【摘要】：目的:胰瘺是胰十二指腸切除術(shù)后的一種嚴(yán)重并發(fā)癥,且發(fā)生率較高,嚴(yán)重影響了患者的健康恢復(fù)。本研究通過(guò)對(duì)我院胰十二指腸切除術(shù)后患者相關(guān)指標(biāo)的統(tǒng)計(jì)分析,探討術(shù)后早期監(jiān)測(cè)指標(biāo)對(duì)胰瘺的預(yù)測(cè)價(jià)值,對(duì)于早期預(yù)測(cè)術(shù)后并發(fā)癥并采取相關(guān)預(yù)防措施意義重大。方法:回顧性分析吉林大學(xué)第二醫(yī)院肝膽胰外科2013年1月到2016年12月116例胰頭十二指腸切除術(shù)患者的臨床資料,根據(jù)國(guó)際胰瘺研究小組提出的胰瘺定義,將胰十二指腸切除術(shù)后前3天血清淀粉酶、引流液淀粉酶、血白細(xì)胞、血清白蛋白及C反應(yīng)蛋白及相關(guān)指標(biāo)的變化趨勢(shì)進(jìn)行單因素分析,多因素分析以胰瘺和非胰瘺作為因變量,胰瘺的相關(guān)預(yù)測(cè)指標(biāo)作為自變量,采用二元Logistic回歸分析。結(jié)果:接受胰十二指腸切除術(shù)患者共116例,其中16例患者術(shù)后發(fā)生胰瘺,胰瘺發(fā)生率為13.8%,死亡2人,胰瘺病死率12.5%,Logistic回歸分析表明術(shù)后第1天腹腔引流液淀粉酶和血淀粉酶水平、術(shù)后第3天血清白蛋白水平是胰瘺的獨(dú)立危險(xiǎn)因素。結(jié)論:胰十二指腸切除術(shù)后第1天腹腔引流液淀粉酶和血淀粉酶、術(shù)后第3天血清白蛋白水平對(duì)胰瘺有重要的預(yù)測(cè)意義。
[Abstract]:Objective: pancreatic fistula is a serious complication after pancreatoduodenectomy. Through the statistical analysis of the related indexes of patients after pancreaticoduodenectomy in our hospital, the predictive value of early monitoring indexes for pancreatic fistula was discussed, which is of great significance for early prediction of postoperative complications and relevant preventive measures. Methods: the clinical data of 116 patients undergoing pancreaticoduodenectomy from January 2013 to December 2016 in the Department of Hepatobiliary and Pancreatic surgery in the second Hospital of Jilin University were retrospectively analyzed. The changes of serum amylase, drainage fluid amylase, white blood cell, serum albumin, C-reactive protein and related indexes were analyzed by single factor analysis 3 days before pancreaticoduodenectomy. Multivariate analysis of pancreatic fistula and non-pancreatic fistula as dependent variables, pancreatic fistula related predictors as independent variables, using binary Logistic regression analysis. Results: there were 116 patients undergoing pancreaticoduodenectomy, 16 of them had pancreatic fistula, the incidence of pancreatic fistula was 13.8. the fatality rate of pancreatic fistula was 12.5. logistic regression analysis showed that amylase and serum amylase levels in peritoneal drainage fluid were found on the first day after operation. Serum albumin level was an independent risk factor for pancreatic fistula on the third day after operation. Conclusion: the levels of amylase and serum amylase in peritoneal drainage fluid on the first day after pancreaticoduodenectomy and serum albumin on the third day after pancreatoduodenectomy are important predictors of pancreatic fistula.
