【摘要】：目的:研究咪達(dá)唑侖聯(lián)合丙泊酚靶控輸注(target controlled infusion,TCI)靜脈鎮(zhèn)靜對拔除下頜第三磨牙患者在圍手術(shù)期焦慮水平的影響。方法:選擇北京大學(xué)口腔醫(yī)院拔除下頜阻生第三磨牙,在初次就診時(shí)狀態(tài)焦慮量表(state anxiety inventory,SAI)得分≥38分的患者,根據(jù)是否于靜脈鎮(zhèn)靜下拔牙分為靜脈鎮(zhèn)靜+局部麻醉組(intravenous sedation,IVS)和局部麻醉組(local anesthesia,LA)。每組患者再根據(jù)初次就診時(shí)的SAI評分分為兩個(gè)亞組:IVS-Ⅰ、LA-Ⅰ(SAI得分38~50)和IVS-Ⅱ、LA-Ⅱ(SAI得分51~80)。根據(jù)患者在初診(T1)、手術(shù)前(T2)和手術(shù)后7 d(T3)時(shí)SAI評分,評價(jià)患者在手術(shù)前后的焦慮情況。通過觀察患者術(shù)中的心率、血壓和疼痛視覺模擬評分(visual analogue scale,VAS)評價(jià)患者手術(shù)中的焦慮情況。結(jié)果:IVS組和LA組在T1、T2、T3時(shí)的SAI評分差異無統(tǒng)計(jì)學(xué)意義(P0.05),IVS組在術(shù)中的心率、血壓和VAS疼痛評分顯著低于LA組(P0.001)。結(jié)論:咪達(dá)唑侖聯(lián)合丙泊酚靶控輸注靜脈鎮(zhèn)靜可以顯著降低拔除下頜第三磨牙患者術(shù)中的焦慮,提高手術(shù)的舒適性,使心率、血壓、血氧飽和度維持在正常水平。但對比兩組患者的SAI評分發(fā)現(xiàn),靜脈鎮(zhèn)靜對初診(T1)、手術(shù)前(T2)、手術(shù)后7d(T3)的焦慮水平無明顯影響。
[Abstract]:Aim: to study the effect of intravenous sedation with midazolam combined with propofol target controlled infusion of (target controlled on the perioperative anxiety of patients with mandibular third molar extraction.
