【摘要】：肝實質(zhì)離斷和出血控制技術(shù)是腹腔鏡肝切除術(shù)中的核心環(huán)節(jié)。目前腹腔鏡下使用的肝血流阻斷技術(shù)包括全入肝血流阻斷、區(qū)域性血流阻斷、肝靜脈阻斷、肝下下腔靜脈阻斷等諸多方法。術(shù)者要結(jié)合自身的腹腔鏡技術(shù)水平、肝切除的范圍和位置、合并肝硬化的程度選擇合適的肝血流阻斷技術(shù),以減少術(shù)中大出血的發(fā)生,提高腹腔鏡肝切除的成功率,減少腹腔鏡下斷肝過程出血及術(shù)后并發(fā)癥的發(fā)生,也可促進(jìn)腹腔鏡肝切除術(shù)的進(jìn)一步發(fā)展及推廣。
[Abstract]:Hepatic parenchyma amputation and bleeding control are the core of laparoscopic hepatectomy. At present, laparoscopic hepatic blood flow occlusion techniques include total hepatic blood flow occlusion, regional blood flow occlusion, hepatic vein occlusion, subhepatic vena cava occlusion and many other methods. In order to reduce the occurrence of massive intraoperative hemorrhage and improve the success rate of laparoscopic hepatectomy, the operative should select appropriate hepatic blood flow occlusion technique in combination with their own laparoscopic technique, the scope and location of hepatectomy and the degree of liver cirrhosis. It can also promote the further development and popularization of laparoscopic hepatectomy by reducing the incidence of bleeding and postoperative complications during laparoscopic hepatectomy.
