【摘要】：目的探討改良荷包縫合法在腹腔鏡闌尾切除術(shù)中的可行性及臨床效果。方法 2012年1月~2014年12月我科在36例腹腔鏡闌尾切除術(shù)中應(yīng)用改良荷包縫合法,即間斷環(huán)形縫合前半荷包與間斷縱形(或斜形)縫合后半荷包合成一個(gè)荷包,包埋闌尾殘端。結(jié)果術(shù)中無(wú)須改變持針?lè)绞胶涂p合習(xí)慣即可輕松完成腹腔鏡下荷包縫合,36例LA均順利收緊荷包縫線并包埋闌尾殘端,無(wú)一例中轉(zhuǎn)開(kāi)腹。手術(shù)時(shí)間(70±20)min,術(shù)后住院時(shí)間(5±2)d。無(wú)荷包縫線撕裂漿肌層、闌尾殘端漏或腹腔殘余膿腫等并發(fā)癥。36例術(shù)后半年通過(guò)電話隨訪,均無(wú)腹痛、腹脹等。結(jié)論改良荷包縫合法縫合荷包包埋闌尾殘端具有操作方便,安全可靠的優(yōu)點(diǎn),避免應(yīng)用傳統(tǒng)方法縫合荷包給手術(shù)操作帶來(lái)的諸多不便,降低因闌尾根部處理不滿意而中轉(zhuǎn)開(kāi)腹的幾率,值得推廣應(yīng)用。
[Abstract]:Objective to investigate the feasibility and clinical effect of modified purse suture in laparoscopic appendectomy. Methods from January 2012 to December 2014, the modified purse suture method was used in 36 cases of laparoscopic appendectomy, that is, the first half purse of discontinuous annular suture was combined with the longitudinal (or oblique) suture of the discontinuous shape, and the half purse was composed of one purse, which was used to bury the appendiceal stump. Results Laparoscopic purse suture was performed easily in 36 cases of LA without changing the needle holding and suture habits. All the 36 cases of LA successfully tightened the suture of the purse and buried the stump of the appendix. No case was transferred to open the abdomen. The operative time was (70 鹵20) min and the postoperative hospitalization time was (5 鹵2) d. No laceration of the sarcoplasmic muscle layer, leakage of appendicitis stump or abscess of abdominal cavity. 36 cases were followed up by telephone half a year after operation. There was no abdominal pain or abdominal distension. Conclusion the modified purse suture method has the advantages of convenient operation, safe and reliable operation, and avoids the inconvenience caused by the traditional method. It is worth popularizing to reduce the probability of conversion to laparotomy due to unsatisfactory management of appendix root.
