【摘要】：[目的]探討弧形骨槽聯(lián)合髕內(nèi)側(cè)骨筋膜覆蓋技術(shù)雙束解剖重建內(nèi)側(cè)髕股韌帶(medial patella femoral ligament,MPFL)治療復(fù)發(fā)性髕骨脫位的中短期臨床效果。[方法]回顧本院2012年2月~2015年10月診斷為復(fù)發(fā)性髕骨脫位的患者25例,其中男11例,女14例,年齡13~35歲,平均20.7歲,手術(shù)以收肌結(jié)節(jié)與股骨內(nèi)上髁最高點(diǎn)連線的中點(diǎn)作為MPFL股骨止點(diǎn)并建立7 mm骨道,髕骨內(nèi)緣1/3做髕骨側(cè)止點(diǎn),使用磨鉆于該止點(diǎn)建立約2 cm的弧形骨槽以充分包埋移植物并用骨筋膜充分覆蓋和固定移植物。將移植物兩游離緣由髕骨側(cè)從股內(nèi)側(cè)肌斜束與關(guān)節(jié)囊之間自股骨側(cè)切口穿出并于骨道內(nèi)口用可吸收界面螺釘固定。臨床評(píng)價(jià)包括手術(shù)前后的Kujala、Lysholm和Tegner評(píng)分,影像學(xué)分析包括手術(shù)前后的髕股適合角(congruance angle,CA),髕骨傾斜角(patellar tilt angle,PTA),髕骨外移率(patellar lateral shift ratio,PLSR)。[結(jié)果]術(shù)后隨訪時(shí)間均在12個(gè)月以上,平均(18.84±4.95)個(gè)月,未見(jiàn)復(fù)發(fā)、感染等相關(guān)并發(fā)癥。所有患者術(shù)后Kujala、Lysholm和Tegner臨床評(píng)分較術(shù)前均有顯著性改善(P0.05)。所有患者影像學(xué)指標(biāo)均恢復(fù)至正常范圍,術(shù)前術(shù)后改變差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。[結(jié)論]弧形骨槽聯(lián)合髕內(nèi)側(cè)骨筋膜覆蓋技術(shù)雙束解剖重建MPFL具有切口小、復(fù)發(fā)率低等優(yōu)點(diǎn),中短期臨床療效滿意。
[Abstract]:[objective] to investigate the short and medium term clinical effect of double bundle anatomical reconstruction of medial patellofemoral ligament (medial patella femoral ligamentum) with arc bone groove combined with medial patellar fascia covering technique in the treatment of recurrent patellar dislocation. [methods] from February 2012 to October 2015, 25 patients with recurrent patellar dislocation were reviewed, including 11 males and 14 females, aged 1335 years with an average of 20.7 years. The midpoint of the line between adductor tubercle and the highest point of the medial epicondyle of the femur was used as the midpoint of MPFL femur, and the 7 mm bone canal was established, and the patellar lateral insertion point was made at 1 / 3 of the inner edge of the patella. An arcuate bone groove of about 2 cm was established by grinding at the insertion point to fully embed the graft and to adequately cover and fix the graft with the osseous fascia. The patellar side of the graft was perforated from the femoral incision between the medial femoral muscle oblique bundle and the articular capsule and fixed with absorbable interface screws at the internal orifice of the bone canal. The clinical evaluation included the preoperative and postoperative Kujalahe Lysholm and Tegner scores. The imaging analysis included the patellofemoral angle (congruance angle CA), the patellar obliquity angle (patellar tilt angle( patellar tilt), and the patellar outward migration rate (patellar lateral shift (patellar lateral shift). [results] all the patients were followed up for more than 12 months, with an average of (18.84 鹵4.95) months. No recurrence, infection and other related complications were found. The clinical scores of Kujalahe Lysholm and Tegner were significantly improved after operation (P0.05). All the imaging indexes returned to normal range, the difference of preoperative and postoperative changes was statistically significant (P0.05). [conclusion] double bundle anatomical reconstruction of MPFL with arc-shaped bone groove combined with medial patellar fascia covering technique has the advantages of small incision and low recurrence rate. The short and medium term clinical effect is satisfactory.
