【摘要】：背景:一項(xiàng)比較物理聯(lián)合藥物與單獨(dú)物理或藥物預(yù)防深靜脈血栓形成臨床效果的薈萃分析顯示,聯(lián)合預(yù)防能夠降低深靜脈血栓形成發(fā)生的危險(xiǎn)性。目的:分析髖關(guān)節(jié)置換圍手術(shù)期深靜脈血栓預(yù)防的有效措施。方法:采取前瞻性研究,將行髖關(guān)節(jié)置換后患者350例隨機(jī)被分為2組,一組應(yīng)用間歇充氣加壓裝置與標(biāo)準(zhǔn)劑量低分子肝素干預(yù)(肝素聯(lián)合IPC組);另一組只接受標(biāo)準(zhǔn)劑量低分子肝素干預(yù)(低分子肝素組)。觀察兩組病例在圍手術(shù)期的深靜脈血栓發(fā)生率。結(jié)果與結(jié)論:(1)肝素聯(lián)合IPC組深靜脈血栓形成發(fā)生率(11%),低分子肝素組深靜脈血栓形成發(fā)生率(33%);肝素聯(lián)合IPC組深靜脈血栓形成發(fā)生率明顯低于低分子肝素組(P0.01);(2)兩組患者均無(wú)癥狀性肺栓塞發(fā)生;(3)結(jié)果表明,應(yīng)用間歇充氣加壓裝置聯(lián)合標(biāo)準(zhǔn)劑量低分子肝素抗凝防治深靜脈血栓較單純應(yīng)用標(biāo)準(zhǔn)劑量低分子肝素效果更好。
[Abstract]:Background: a meta-analysis of the clinical efficacy of physical combined drugs versus physical or drug alone in the prevention of deep venous thrombosis has shown that combined prevention can reduce the risk of deep venous thrombosis. Objective: to analyze the effective measures to prevent deep venous thrombosis in perioperative period of hip arthroplasty. Methods: in a prospective study, 350 patients after hip replacement were randomly divided into two groups. One group was treated with intermittent inflatable pressure device and standard dose low molecular weight heparin (heparin combined with IPC group), the other group only received standard dose low molecular weight heparin intervention (low molecular weight heparin group). The incidence of deep venous thrombosis was observed in both groups during perioperative period. Results and conclusion: (1) the incidence of deep venous thrombosis in heparin combined with IPC group (11%), low molecular weight heparin group (33%), and the incidence of deep venous thrombosis in heparin combined with IPC group was significantly lower than that in the low molecular weight heparin group (P0.01); (2). No symptomatic pulmonary embolism occurred in all patients. (3) the results showed that, The effect of intermittent inflatable pressure device combined with standard low molecular weight heparin on the prevention and treatment of deep venous thrombosis was better than that of standard dose of low molecular weight heparin alone.
